I used @claudeai to build an interactive tool called AngioView (angioview.com) to help you understand coronary angiograms. One of the most challenging parts of being a first-year cardiology fellow is learning how to interpret cath lab imaging. It can take a lot of repetition before you're able to create a mental 3D map of coronary anatomy, which is so important because you need to understand how changes in cranial/caudal and LAO/RAO angulation affect the overall coronary perspective. I never found a high-quality tool out there to help visualize all this (at least not for free) so hopefully this site helps speed up that learning process!

🫀Ventriculo-arterial coupling in septic shock: better cardiac efficiency does not necessarily mean better tissue perfusion One of the fundamental assumptions in septic shock resuscitation is that improving cardiac performance will restore peripheral perfusion. But is this physiological relationship as predictable as we believe? Thisnarticle explored the question in a prospective observational study during the first six hours of septic shock resuscitation. Left ventriculo-arterial coupling (LVAC) describes the interaction between arterial load (Ea) and ventricular end-systolic elastance (Ees): LVAC = Ea/Ees An elevated ratio indicates that arterial load is excessive relative to ventricular contractility, potentially compromising the efficiency of mechanical energy transfer. The investigators defined uncoupling as LVAC >1.36 and examined its relationship with capillary refill time (CRT). The results challenge a purely macrocirculatory approach. Patients transitioning from uncoupled to coupled exhibited a 33.8% reduction in CRT, accompanied by increased stroke volume, stroke work and ventricular efficiency. Conversely, patients developing uncoupling experienced a 17.5% prolongation of CRT. However, the overall correlation between changes in LVAC and CRT was weak and statistically nonsignificant Perhaps the most interesting finding concerns fluid responsiveness. In initially uncoupled patients, fluid administration increased stroke volume by 14%, yet CRT did not significantly improve. Dobutamine produced a comparable stroke volume increase of 14.2%, but was associated with a 26.2% reduction in CRT. This distinction is physiologically important. Increasing preload and increasing contractility can produce similar changes in stroke volume while exerting different effects on ventricular energetics, arterial interaction and peripheral perfusion. Nevertheless, these observational findings cannot establish that dobutamine is superior to fluids. The interventions were not randomized, and their clinical indications differed. What does this mean at the bedside? Fluid responsiveness identifies the capacity to increase cardiac output following preload augmentation. It does not establish that additional flow will improve tissue perfusion. Similarly, restoring ventriculo-arterial coupling may improve cardiovascular mechanical performance without necessarily correcting microvascular dysfunction. In septic shock, endothelial injury, heterogeneous capillary flow, altered vascular reactivity and impaired oxygen extraction may persist despite apparently favorable macrocirculatory changes. This study reinforces the importance of integrating echocardiography, arterial pressure, fluid responsiveness and peripheral perfusion rather than treating any single parameter as the ultimate resuscitation target. The website is live at intro2echo.com and the github is at github.com/douyang/echodemo It was really fun to make the free movement - the video doesn't do it justice, but you can free move both the TTE and TEE probe and it'll generate the 2D image in according to the 3D model. There's a couple of chapters on common echo concepts and common diseases. Would love feedback!
I'm almost ready to share it, but Claude Code is very good at keeping consistency across views and chapters. For example, I can specify a heart phenotype, and will make all the images consistent and physiologically plausible. Even with unlimited time, I don't think I could create such a nice tutorial!

Albuminuria and heart failure UACR AND HEART FAILURE: A CARDIORENAL WARNING SIGNAL Albuminuria is more than a marker of kidney damage. It is an independent predictor of incident and worsening heart failure, hospitalization and mortality. 📊 Key messages: 🔹 HF risk rises progressively with increasing UACR—even at low levels of albuminuria. 🔹 Across multiple studies, albuminuria is associated with approximately 1.5- to 6-fold higher HF risk, depending on the population and UACR level. 🔹 Its prognostic value extends beyond kidney disease, including in patients with diabetes and atrial fibrillation. 🔹 UACR is a simple, accessible biomarker that can improve cardiorenal risk stratification. 💡 Clinical takeaway: Don’t wait for eGFR to decline. Measure UACR to identify patients at increased cardiovascular and HF risk—and act early. #HeartFailure #Cardiorenal #Albuminuria #CKD #Cardiology #Nephrology #UACR #CardiovascularPrevention @JavedButler1 @mvaduganathan @gcfmd @SJGreene_md @GianluSava @HFA_President @hfcollaboratory academic.oup.com/eurheartj/a…

There is a hierarchy in diastolic function parameters. Not all parameters have the same power. E/A < 0.5 is always grade 1 with NL DFP. E/A around 0.6 most likely NL DFP. E/e' of 8 says normal diastolic filling pressure. Since the patient has SOB, I would exercise him with filling pressure assessment. PV Doppler pattern wins over LARS when there is a discordance.
A diastology case. 81 F SOB with SOA. LV EF normal no LVH in SR. BMI 44 e’ lateral 9 & septal 6 E/e 8 PV S/D=1.37 E/A=0.62 TR 2.85 LARS 20% Biplane LAVI 36 ml/m2 Normal IVC If i correct LAVI to BSA for BMI 30 its 44 ml/m2 Whats the LAP normal or high 🤷‍♂️ if i put in guidelines get grade III with ⬆️LAP but if i trim the TR to 2.7 & LAVI to 34 its normal. Seems like the partitions are too tight? LARS 20 i want to say LAP normal #echofirst #diastology @argulian @echo_batman @iamritu @alex1708ander @VerwerftJan @StellEkaterina @PWesslyMD
