And I still can’t get a summary of a patient chart…. Using control-F to see if I can locate the rogue EMG hiding somewhere….
GPT-6 Astra just solved a problem medical students have been facing for decades Medical students have been working around the same three problems for decades. Someone talked to them, identified all three, and built a service with Astra that solves them in one tool. The problems are structural. Flat anatomical diagrams strip depth out of organs entirely. CT cross-sections don't connect naturally to the overview diagrams students spend years memorizing. And reading a scan requires holding three mental models simultaneously - patient orientation, cross-section direction, and how every organ shape shifts through each layer. Astra built a service that handles all three in one place. Not a better textbook. A spatial tool that connects the diagram, the CT view, and the 3D relationship together. Medical education hasn't changed its core format in over a century. This might be the first tool designed around how anatomy is actually understood - not just displayed.

Luigi Mangione has admitted killing UnitedHealthcare CEO Brian Thompson. On December 4, 2024, the morning Thompson was killed, UnitedHealth Group traded above $600 a share. The company was preparing to tell investors it expected: $450 billion to $455 billion in revenue. $29.50 to $30.00 in adjusted earnings per share. $32 billion to $33 billion in operating cash flow. Twenty months later, Mangione admitted pulling the trigger. UnitedHealth closed yesterday at $401.73. Down roughly 34 percent from the day Thompson was killed. But do not make the mistake of attributing the decline to Mangione. UnitedHealth broke on its own economics. Inflated medical costs exceeded its assumptions. UnitedHealthcare’s operating earnings fell 40 percent. Optum Health went from a $7.8 billion operating profit to a loss. The company withdrew its guidance. Its CEO resigned. At the bottom, the stock had fallen approximately 62 percent from its November 2024 peak. UnitedHealth has since staged a real rebound. But it repaired its margins by raising prices, reducing benefits, exiting underpriced markets, blaming physicians, and preparing to lose roughly 1.1 million Medicare Advantage members. That is the part Wall Street understands better than Washington. UnitedHealth did not recover by making the machine work for more people. It is recovering by removing people for whom the machine no longer worked profitably. Mangione attacked the symbol. The market exposed the economics.

I can't wait for ambient AI to show us all how much of physician notes have been fradulent. Many doctors who refuse to document things which never happened are threatened by administrators. "You need to document a medication review and review of systems." Those admin point to the "good doctors" who make sure and document that every time. Time for ambient AI to tell us who is actually doing what they are documenting.
Is always-on ambient AI - i.e. AI that hears and sees everything - the "holy grail" of healthcare? Or a dystopian surveillance state that will scare off human clinicians altogether? Recently a family member saw a doctor. No AI scribe - just a good ol' human clinician doing normal medicine like a sane person 😉. After, we checked the patient portal and noticed things in the note that did NOT actually happen: → "1 week of symptoms"... told them it was a 2 day history → "Surgical history reviewed - none pertinent"... nope, was never asked → "Pupils are equal, round and reactive to light"... wait, when did an eye exam happen? Did any of this materially impact the diagnosis or management of the patient? No. Would any of this staying in the record actually matter? Probably not… unless "no surgical history" got copied/pasted and people stopped asking. I suspect this clinician went on auto-pilot with smart phrases, needing boxes checked for "compliance" and "documentation." The most charitable read is that they genuinely misremembered. You'd think AI scribes fix this, but they don’t completely - not just yet. Because even in the age of AI scribes, humans are still in the loop - they edit the note and "verify" the AI output. Humans still "verbalize" the physical exam so the AI scribe even knows one happened. Which means if a clinician skipped something needed for compliance, they could just say they did it - and we just moved the loophole from paper to software. The fix, in theory, is visual ambient - cameras everywhere with computer vision AI. One day we won't need to tell the AI scribe we did an exam - it'll just watch us do it. Yet what happens if the AI scribe sees we skipped something, hears we didn't ask something - but the clinician says "yes I did"? What ethical responsibility does AI have to monitor and report what humans do? Should we care? On one hand, I dislike the idea of a surveillance state in healthcare - cameras and microphones recording all the time gives me the creeps. On the other, there's the bull case for always-on Ambient AI. We'll catch falls and deterioration faster, and we’ll measure vitals without extra hardware. It'll be a huge deterrent for bad actors - not the people bad at documentation, but clinicians who truly harm patients. Always-on AI will weed out these bad humans faster. But amid all that upside, this surveillance and documentation baggage comes with it - whether we like it or not. Maybe none of this will matter. The AI is always there on a Star Trek ship - and no one there seemed spooked by it. Most likely it'll be normalized and clinicians will be fine with it… but should we? And if the price of finally knowing whether someone truly did a physical exam is Big Brother standing over us in the room - is that a price we're willing to pay? In the era of AI, we’ll quickly stop asking whether something is possible - and think more about whether it should.

Seizing another nation's assets to pay for unrelated future claims is an incendiary precedent. Those who celebrate or profit from such funds should remember: once governments normalize confiscation, no one's assets are safe. Ensuing chaos will not be pretty or peaceful.

This is good news, but the fight is not over. The Senate just rejected the NDAA that would merge our military with Israel’s, which will now proceed to the House. Call your senators & representatives and tell them not to support the NDAA until the merger w/ Israel (Section 219) is removed. The senate switchboard number is: (202) 224-3121
US Senate Democrats block $1.15 trillion defense bill over Iran war objections, @ReutersZengerle reports (Reuters) - U.S. Senate Democrats blocked a $1.15 trillion annual defense policy bill on Tuesday, citing frustration with the Iran war and President Donald Trump's failure to consult with Congress about his decision to send U.S. forces into the conflict. "Trump started this war without authorization, without a strategy, and without an exit," Senator Chuck Schumer of New York, the Senate's Democratic leader, said in a Senate speech announcing he would vote no. The procedural vote on the National Defense Authorization Act, or NDAA, failed. Even though the yes votes outnumbered no votes by 50-46, the bill fell short of the 60 votes needed to move ahead in the 100-member Senate. The vote was along party lines. Every Republican voted in favor of moving ahead except for the chamber's Republican majority leader, John Thune of South Dakota, who voted no under Senate rules only in order to enter a motion to reconsider. The bill had been expected to stall, after nine Democratic members of the Armed Services Committee voted against the NDAA when the panel considered it last month. The NDAA typically passes with strong bipartisan support. Democrats are concerned that authorizing a huge Pentagon budget would be seen as approval of the war on Iran that began with strikes by the U.S. and Israel on February 28. They also object to the Trump administration's push to increase military spending to a total of $1.5 trillion this year, including several hundred million dollars he has requested in a separate spending bill, even as social programs for Americans are being cut.

Update on NDAA - Action Needed the National Defense Authorization Act (NDAA) contains clauses that seek to merge the US military and intelligence apparatus with Israel, with no viable way for any administration to reverse it. In doing so, the US would be ceding its sovereignty to a foreign state, which will have TOTAL access to all US intelligence, military hardware and software, along with a $1.5 TRILLION discretionary budget. Benjamin Netanyahu called this clause insertion into the bill as "my plan," suggesting it was written at Israel's behest. Take a moment to call your representative. ☎️ *202-224-3121* 🗣️ As a constituent, I'm calling to ask your rep to: - Vote NO to FY27 NDAA bill - Demand the military integration clauses be removed and the SAVE Act taken out from the FY27 NDAA bill and voted on separately - Vote NO to FY27 NSRP bill unless the clause to continue $3.3 billion annual military aid to Israel is removed per the amendment by Rep Massie. - Ensure funds are redirected towards the needs of American families.

Trump believes he is threatening Iran with destruction, but it is America that now stands in danger. If he attempts to eradicate Iranian civilization, the United States will no longer be viewed as a stabilizing force in the world, but as an agent of chaos—effectively ending our status as the world’s greatest superpower. This would upend our economy and shatter the global order. The process is already underway, yet we still have time to avert catastrophe if Trump finds the courage to pursue serious negotiations rather than reckless rage and destruction.

Trump’s negotiations were working. Thats why Israel forced this war by attacking Iran, to sabotage the deal he was about to make. Israel needs this war to reach their goals. We need peace to achieve ours. Whoever restores order will hold the power. This should be our goal.
