As part of #SocialPrescribingWeek we would like to introduce two key Social Prescribing link workers @PeeerYouth who work tirelessly behind the scenes to support our young people. Sarah Shameti has been at the forefront of @RProgramme referring #CYP with her team @SotonChildHosp

We encourage young people living with long term health conditions to act as positive role models for other #CYP faced with similar issues. Here is our wonderful youth ambassador @AmeliaShipton working with some of the young women from @SotonChildHosp through sensory Theatre 💚

Through hybrid & in-person sessions we have supported #CYP to access diverse opportunities. We have reached young people living with complex long term health conditions and who have also faced isolation through the power of #SocialPrescribing @NASPTweets #SocialPrescribingweek
