The entire heart can be adequately perfused by a single patent graft. The internal thoracic artery provides the most durable inflow source we know.
Surgeons at @CleClinicHVTI found that patent bypass grafts can sustain survival in patients even after ostial occlusion of all epicardial coronary arteries. Read the editor's choice article for free in #JTCVS until 10/30: hubs.la/Q04z3NFV0

1-There was no CABG arm
2-This was not a CABG population to start with:
-Very low SYTAX score=17
-Majority presented with ACS
3-All-cause mortality at 1yr was x3 that of FAME III
4-Don’t understand the rationale for comparing CABG vs PCI. It has no relevance to this trial

Today, we lost a surgical legend. A medical statesman, a friend, a man of tremendous integrity who placed evidence-based patient care above personal gain. This video which David shared with me a few months ago offers a glimpse into his inspiring story❤️🙏🏻youtu.be/fH0guNXZN5c?is=e0CD…

Surgical societies should maintain operational and financial independence to effectively advocate for patient safety, deliver unbiased clinical guidelines, and support fair, evidence-based surgical training without conflicts of interest.
Maintaining this independence is essential for several critical reasons:
1. Unbiased Clinical Standards: Independence ensures that research, innovation evaluations, and clinical practice guidelines are free from commercial bias or pressure from medical device and pharmaceutical industries.
2. Objective Training & Education: It allows surgical trainees and established professionals to engage in objective mentoring, continuing medical education, and advocacy without being tied to the political or administrative agendas of specific hospitals or health systems.
3. Stronger Patient Advocacy: Autonomous societies can lobby governments and healthcare regulators objectively, directly addressing systemic challenges such as surgical waitlists, workforce wellbeing, and equity in care delivery.
4. Accountability: Independent bodies—such as third-party surgical research and audit panels—are better positioned to enforce rigorous quality improvements and transparent system performance

Cardiac surgeons are trying to compete with cardiologists on convenience.
They're going to lose.
A smaller incision doesn't make it minimally invasive.
It makes it minimal access.
And a mini-AVR?
It actually hurts more than a standard sternotomy because you open the sternum AND break into the rib cage.
"If you think you can compete convenience-wise with a cardiologist who does stents when you do a CABG and your incision is small, you're not fooling anyone."
The game surgeons should be playing isn't incision size.
It's durability. Longevity. Freedom from re-intervention.
That's the competition cardiologists can't win.
But surgeons keep fighting on the wrong turf.
