We have spent an brilliant day at Liberton High School and Broughton High School in Edinburgh today, inspiring the next generation of surgeons!
With lots of opportunity to take part in surgical simulations, including lap skills, stapling and VR as well as a chance to chat to our faculty, we hope to see some pupils following in our footsteps soon 🩺
A massive thank you to @CMRSurgical, @RCSEd and @MedtronicUK for supporting today's visit!

Training logbook numbers by specialty relative to 2019. Millions fewer procedures performed with/by a trainee. Consultants - remember that you were all trainees once. Time to train the next generation! Every case is a training case. #NoTrainingTodayNoSurgeonsTomorrow

November is 💜#PancreaticCancerAwarenessMonth 💜and last thursday was🟣#WorldPancreaticCancerDay🟣!!
So lets have a look at our recent paper focusing on 🏺periampullary cancer, a heterogenous group including PDAC, distal CCA, and duodenal cancer:
➡️ doi.org/10.1093/bjsopen/zrad…
Tumour origin, diagnostic accuracy and histopathological evaluation in patients with periampullary cancer: nationwide cohort study
The proportion of non-pancreatic periampullary cancer is substantially higher in patients with a histopathological diagnosis than in those with a clinical diagnosis. Histopathological standardization correlated with an increased frequency of pancreatic ductal adenocarcinoma diagnosis, R1 resection and N1 stage. The misdiagnosis of periampullary cancer origin occurs in a clinically relevant proportion of patients and needs to be considered in patients with a neoadjuvant oncological treatment plan.

Intraoperative Findings:
🟢Chronic cholecystitis
🟢Cholecystoduodenal fistula
🟢Duodenum adhered to Hartmann's pouch
🟢Sufficient dissection of hepatocystic triangle not feasible to achieve critical view of safety
Surgical Approach:
🔴Judicious use of diathermy
🔴Leave disc of posterior GB wall on duodenum
🔴Sharp dissection after visual confirmation of correct planes
🔴Divide GB
🔴Inspect remnant GB stump for any stones
🔴Utilise IOC +/- IOUS for anatomical confirmation if needed
🔴Choledochoscopy to rule out ductal stones
🔴2/0 Vicryl absorbable continuous suture to close remnant GB
🔴2/0 Vicryl absorbable interuppted suture to close fistula with omentum over the top
🔴2 x 18F Robinson's drains in subhepatic space
Key Points:
🔵Utilise bailout options (subtotal) in cases where CVS can not be achieved
🔵Seek 2nd opinion if available
🔵Options for subtotal include reconstituting or fenestrating
🔵Advanced lap skills and biliary experience critical➡️close and refer to HPB specialist unit if in doubt
