I propose:
...It is a narrative that often highlights levels of understanding despite the challenges of serious mental disturbances and should be incorporated into clinical training.
... but the relationship between (compliance and insight) is not straightforward and not be considered circular.
6. A more nuanced description of insight emerges when we look at what psychiatrists and others mean when they talk about insight....
4. A simple definition of ‘lack of insight’ (like delusion) is difficult if not impossible to achieve due to the requirement to contextualise such mental phenomena in question within a social and cultural milieu.
5. Compliance with treatment may follow from insight into illness..

3. Part of the construct is a (meta)cognitive style which when lacking, tends towards excessive certainty and rigidity.

1. It does not fit with a narrow version of the ‘medical model’, so let us use a medical model which is deeper and has real explanatory power./

the True Clinician treads a careful but firm line:
a) humbly avoiding imposing a paternalistic, insight-denying, narrative on patient;
b) firmly avoiding colluding with lack of insight in the name of a maternalistic validation of patient's narrative.
