You don't even fight him in the Phantom Liberty ending, so it's unclear if V beating Smasher is canon.
There's also the significant possibility that there are multiple copies of Adam Smasher, and you only beat one of them.
The Edgerunners Mission Kit mentions that there being multiple is an in-universe theory, and that'd be a great way to handle such an important legacy character that you can either kill or spare. It'd also be prefect for him. He'd achieve all machine status.
