Rare UC subtypes, two largest series to date, one message: morphology still predicts behavior. 🔬 Giant cell UC-nuclei ~12x larger than neighboring tumor cells, enriched for T3/4, worse OS PMID: 42252958 🔬 Lipid-rich UC-MDM2(+) could be a pitfall PMID: 42596534 @AJSPJournal

📢 Introducing DermPath in 15! I'm launching a new live series: quick, focused 15-minute dermpath case sessions, straight to the point. 🔴 Live with me, 2:00 PM Eastern, three Mondays. Register for each below: 🗓 Monday, Aug 10 → dermpedia.org/programs/live-… 🗓 Monday, Aug 17 → dermpedia.org/programs/live-… 🗓 Monday, Aug 24 → dermpedia.org/programs/live-… Here's the deal: this is a test run. If turnout is strong, my plan is to keep it going EVERY week, year-round. And that decision rests on how many of you join us live. So if you can be there, please show up! Your presence shapes what happens next. Can't make it live? No problem, especially for my international friends catching this in the middle of the night. Register anyway and I'll make sure you get the recording. Hope to see you Monday! 👇 #pathology #pathologists #pathTwitter #dermpath #dermatology #dermatologia #dermtwitter #dermx #pathx #dermpathx #dermpedia

The photomicrograph shows a distended tubule with an intratubular crystalline cast with agryophilia. While crystalline casts can be seen from calcium phosphate, calcium oxalate and others, the presence of agryophilia on sliver stain is characteristic for 2,8-Dihydroxyadenuria (DHA) crystals. 2,8-DHA is a rare, autosomal recessive trait caused by a genetic deficiency in adenine phosphoribosyltransferase (APRT). Accurate diagnosis is critical (especially in those with limited chronicity) as treatment exists that can ameliorate renal damage in some patients. And, in patients with undiagnosed disease who undergo transplant, the disease will reoccur unless pharmacologic intervention is initiated. Of note, agryophilic, birefringent crystals can also be seen in patients exposed to triamterene which should always be excluded before a diagnosis of 2,8-DHA is rendered. #renalpath #kidneypath #pathology #renal #pathtwitter

New entities in soft tissue pathology ***FET-CREB fusion sarcomas*** Variable histology with epithelioid/round cells showing an odd combination of multiple aberrant markers (CK, EMA, MUC4, ALK, CD30, synapto , etc.) is valuable for screening FET-CREB sarcomas Half of cases are intra-abdominal Wide age range Highly aggressive Will be included in the upcoming BST WHO book FET-CREB is an umbrella term for fusions between FET genes (mainly EWSR1, sometimes FUS) and CREB family transcription factors (CREB1, ATF1, or CREM) Pitfall: Angiomatoid fibrohistiocytoma (AFH) and FET-CREB sarcomas have a similar genotype, but morphology differs and, most importantly, AFH mortality is >1% versus >50% in FET-CREB sarcomas Dr. Agaimy #USCAP2026 #pathology #PathX #PathTwitter

Happy to see our @Pathoutlines article on PATZ1-rearranged sarcoma published 👇🏼 pathologyoutlines.com/topic/… 3 things to remember about PATZ1-rearranged sarcoma: 💎Pseudoalveolar/(micro)cystic change->Swiss 🧀-like appearance 💎Hyalinized blood vessels 💎Rhabdomyoblastic differentiation often seen -- skeletal muscle marker expression tends to be accentuated around the pseudoalveolar spaces Check out our article online (link above) 😊 #PathTwitter #BSTPath
