📚 Previously published in PDI | How did PD peritonitis rates change in Australia after COVID-19 onset? ANZDATA registry: 0.30 vs 0.35 episodes per patient-year, post- vs pre-COVID-19 (p < 0.001).

In human peritoneal mesothelial cells, PolyCore — a glucose-free bimodal PD solution (icodextrin 4%, xylitol 1.5%, L-carnitine 0.02%) — kept barrier resistance comparable to control, higher than icodextrin 7.5%.

The paper also sets out EXCITE, a phase II, single-arm, prospective pilot trial of PolyCore in 28 adults with kidney failure on incremental PD, assessing safety, adequacy, ultrafiltration, solute removal, metabolic parameters and patient-reported outcomes.
The authors conclude PolyCore showed favorable in vitro biocompatibility versus icodextrin 7.5% and may represent a glucose-free bimodal strategy for PD; EXCITE will provide its first clinical assessment of safety and preliminary efficacy.

A 20-year single-centre case series of Stenotrophomonas maltophilia PD-related peritonitis in Hong Kong: 28 episodes in 27 patients, 21.4% medical cure, 28.6% peritonitis-associated death.

Relapsing peritonitis occurred in 9 episodes (32.1%). PD catheter removal followed in 15 (53.6%), and 12 (42.9%) required transfer to haemodialysis.
The authors conclude S. maltophilia was an uncommon cause of PD-peritonitis but conferred substantial adverse outcomes, and that early identification and sufficient duration of appropriate antimicrobials may help achieve better outlook.

A single-centre retrospective cohort of 232 adults on PD in Senegal, 2004–2020: peritonitis at 0.97 episodes per patient-year; technique survival 86.1%, 49.8% and 25.3% at 1, 3 and 5 years.

Permanent transfer to HD was the leading cause of PD discontinuation (59.6%), followed by death (37.7%). Median patient survival was 31 months; median time on PD was 35 months.
The authors conclude PD in Senegal is suitable but challenged by infectious, mechanical, metabolic complications and socioeconomic barriers, with infection control, monitoring, catheter placement and insurance coverage key to equitable access.

9/10 On intraperitoneal amino acids: "We do not recommend intra-peritoneal amino acids as a stand-alone treatment for the prevention or management of PEW." (1D)
10/10 The authors' recommendation on dietetic care: "We recommend individuals receive medical nutrition therapy from a registered dietitian or equivalent on initiation of PD and then at least on an annual basis" (1C)
