In #HFpEF, prominent #PAWP V waves may identify a high-risk LA myopathy phenotype. Among 1,069 patients undergoing invasive exercise testing, 21% had prominent V waves and showed: 📍 Larger #LAVI: 42.2 vs 32.5 mL/m²
📍 Lower LA reservoir strain: 16.4% vs 23.2%
📍 AF: 61.3% vs 22.9%
📍 PA systolic pressure: 54 vs 38 mmHg
📍 Lower PA compliance: 3.03 vs 4.90 mL/mmHg V-wave correction reduced apparent combined pre/post-capillary PH from 60.2% → 29.3%. Prominent V waves were associated with worse RV function/exercise reserve and 2× higher risk of HF 🏥 or ☠️ (adjusted HR 2.09). ahajournals.org/doi/10.1161/… @CircAHA

🫀Is ICI myocarditis a permanent "never again"? For years, ICI myocarditis has meant permanent discontinuation of ICI therapy, but this is increasingly difficult to defend. ICYMI, this #JACCCardioOnc primer explores ICI rechallenge. 📍In a prospective cohort, 14 of 138 patients with definite ICI myocarditis resumed ICI. Only 1 developed recurrent grade 3 myocarditis. None developed severe heart failure, and troponin rises were usually modest. 📍 Note that the cohort was small, highly selected, and managed at a quaternary center. 📍 The authors call for a practical framework: confirm oncologic need, confirm cardiovascular recovery, plan the restart (ultra-low dose, weekly labs and ECG), and define interruption/rescue parameters. Bottom line: ICI rechallenge should be cautiously considered in specialized cardio-oncology programs with a protocol.
