Please stop reporting small changes in GFR like they're meaningful. 45 to 40 is the same number. Just stop telling patients about GFR altogether. Thank you for coming to my TED Talk.

Not a new shooter or a new competitor, but getting more serious about both and trying to make the most of the time I get at the range. First USPSA match on a while today. If I had one take home point it would be a full speed practice run is gold. Slow motion finger guns are fun but full speed is a different game entirely. #uspsa

Why is the media so silent on this ?
Friends! As you know I use this platform to post new science in the field of cardiology. My people in Iran are suffering the darkest time in their history right now. I will use this platform to be the voice of voiceless people while the legacy media are dead silent. Please help to amplify

There still hasn’t been a reckoning for the Joint Commission’s contribution to the opioid crisis. In 2001 they insisted that pain be added as the fifth vital sign, reported along with blood pressure, heart rate, etc. They followed that up with a statement that “therapeutic use of opiate analgesics rarely results in addiction.” Yet after these extreme blunders, costing millions of lives, they are still the accrediting agency for most US hospitals. Keep that in mind the next time they scold you for having a drink at the nurses’ station or failing to include a full ROS in your H&P.
Stunning graph and a stunning quote: "About 1/4 of all deaths for those under 55 recently are overdoses." Oxycontin really was the premiere big pharma disaster. It's hard to think of anything more harmful.

We in the medical profession should be reflecting on why it has become necessary to state the obvious. How did this happen? What are the root causes?

If you think the cards aren’t stacked for corporations, remember that this year CMS (Medicare and Medicaid) proposed 2.9% increases for payment to ASCs (Ambulatory Surgical Centers mostly owned by large health corporations) and 3.4% increases to hospitals (unlawful for physicians to own a hospital). All while proposing 2.83% cuts to physicians. Again. After many years of the same increases and cuts. Remember this when you can’t find a good doctor, but entering a hospital or ASC that looks like a 5 star hotel. It takes much less schooling, debt, knowledge and you get much more free time and more income if you go into healthcare administration rather than healthcare itself.

Can #IndyCar ditch the hybrid and make the cars sound like this. We want people 5 miles away to know there’s an IndyCar race happening

I keep seeing this idea that doctors are idiots about nutrition and that all we do is push pills, instead of curing the real root cause of disease which in this worldview universally comes down to poor diet and exercise. First, I discuss nutrition every day. My goals are not as lofty as these people imagine; I am trying to get my patients to drink water, any water at all, ever. If that succeeds, I try to get them to eat actual food sometimes, not solely food products. Things that occur in nature and do not require a chemistry lab to produce. That’s it. 99% of the people I see don’t get past that. I don’t need to get into details about beef tallow and turmeric and ashwaganda or whatever. I admit I am not an expert in nutrition but I am much more of an expert than 99% will ever need me to be. Second, I don’t get paid to prescribe. I’m hourly. I have no interest in pushing pills. I’d be fine if no one was on them. But I do have to treat the diseases that people already have, even if they were all a 100% direct result from lifestyle (which they aren’t). What would you have me do for the patient who drinks nothing but Mountain Dew and puts it in her children’s baby bottles? Start talking about macronutrients as if that will prevent her impending DKA? Sometimes you need to push some pills. IV meds even.

Board certification used to be a honor, not a mandatory requirement, but it has evolved that way American Board of Internal Medicine sends us this "friendly" letter: pay or perish #medtwitter

The only @IndyCar Jersey I own is an @AlexanderRossi @ArrowMcLaren one. At the time I thought it was a safe purchase.

Well, another interesting episode of @AskOffTrack incoming. I'm not sure if @ArrowMcLaren has noticed that @AlexanderRossi is their top performer. Their loss I guess.
Arrow McLaren announces Christian Lundgaard is set to drive the No. 7 VELO Arrow McLaren Chevy beginning in 2025. Looking forward to welcoming you to the team, @lundgaardoff! 👊
