Great trial with impressive results. The impression is that most included lesions were rather severe so to some extent the trial also seems to say that a negative FFR in a high grade lesion is not as much a guarantee that PCI deferral is fine as previously thought.

1/10 Aortic stenosis pts have discordant high-gradient AS (DHG-AS) with MG ≥40 mm Hg & AVA ≥1.0 cm2
DHG AS survival & level of calcium same as concordant severe AS so don’t delay referral for TAVR/SAVR
DHG-AS= “pseudo-moderate” AS, AVA ≥1.0 cm2 under high flow conditions could actually be <1.0 cm2 at standard flow 250 mL/s ie severe concordant AS as seen in TOPAS 📝
DHG-AS LVOT is not circular it’s elliptical & this larger LVOT results in AVA overestimation

Fellows watch it carefully. Catheter is against the inferior wall pushing the proximal RCA. If you see this either withdraw or exchange with different catheter. Watch how catheter moves towards the Shepards hook dissecting the RCA. Published in PCR.
