Bravo @VaibhavModgil and team!
Good reminder that most of the old fears have not held up. Current data does not show higher prostate cancer or cardiovascular risk when TRT is managed.

Give a man hormone therapy before prostate surgery and his pathology report comes back better. That part isn't in question.
1,099 men, one centre. A clean result meant tumour confined, margins negative, nodes clear, PSA undetectable afterwards. 42% of the hormones-first group got there, against 24% of the men who went straight to surgery.
Then they followed only the clean ones. Three years on, 81.5% of the hormones-first men were still free of rising PSA, against 93.6% of the others.
Before anyone reads that as harm: once the authors matched on baseline risk, the gap stopped being significant. The men offered hormones were sicker to start with. Which is the actual finding. Androgen deprivation shrinks the tumour, clears the margin and flattens PSA, so a high-risk cancer can hand you a low-risk-looking report. The report quietly stopped measuring what it used to measure.
Baseline PSA above 20, Gleason 8 to 10, cT2c or worse still predicted recurrence either way.

When radical nephroureterectomy means dialysis, can we safely preserve the only kidney?
Our latest multicentre study in 62 patients with UTUC and a solitary kidney evaluated long term outcomes after endoscopic kidney sparing surgery.
-Median follow up: 65 months
-73% never required dialysis
-Only 26% ultimately underwent RNU
-5 year cancer specific mortality: 20%
-Mean eGFR decline: 2.9 mL/min/1.73 m²/year
Recurrence was frequent, 70% at 5 years, highlighting that eKSS is not a one time treatment, but a long term disease management strategy requiring rigorous endoscopic surveillance.
👉 In carefully selected patients with an imperative indication, preserving the kidney can preserve much more than renal function.

Long live DRE, die DRE ☝️
DRE may be dying as a screening test, but it is far from dead. The latest evidence supports removing DRE from routine screening in asymptomatic men, yet it still retains an important role in clinical staging until imaging fully replaces it

Scientists have identified specific gut bacteria that appear to trigger multiple sclerosis (MS).
In a groundbreaking study conducted at Ludwig Maximilian University of Munich, researchers examined 81 pairs of identical twins in which only one sibling had MS. This unique design allowed them to control for genetic and environmental factors, isolating the role of the microbiome.
The team found that two bacterial species, Eisenbergiella tayi and Lachnoclostridium, were significantly more abundant in the twins with MS. When these microbes were transferred into mouse models, they directly induced MS-like autoimmune symptoms, providing strong causal evidence.
This is the most precise identification of microbial triggers for MS to date and adds powerful support to the gut-brain axis in autoimmune disease. The discovery raises hope for new approaches to early detection, prevention, and treatment — potentially by targeting or modulating these specific bacteria before symptoms appear.
While human clinical trials are still needed, the findings represent a major step toward microbiome-based therapies for MS and other autoimmune conditions.

Pattern recognition: Testicular Sonography.
I have spent the past few weeks perfecting my knowledge of testicular sonography, and I'll be sharing some of the most important patterns you should know in three parts.
Here's part 1. 👇👇
Contributions and corrections are welcome.
Quiz: What's the difference between an epididymal cyst and spermatocele on ultrasound?
7,000 false positives per square millimeter. The culprit was the lab gloves.
University of Michigan researchers just upended a core assumption in microplastics science. Latex and nitrile gloves, worn by the scientists doing the measuring, shed stearate particles that look chemically identical to polyethylene. Standard infrared and Raman instruments can't tell them apart. The gloves were counting as plastic.
Seven glove types tested. All contaminated. The cheapest fix: switch to cleanroom gloves, which dropped false positives to around 100 per mm² vs. 7,000.
The "credit card per week" headline (5 grams, WWF/Newcastle 2019) has separate problems. A 2022 re-analysis found severe methodological errors in the original estimate. Actual measured intake is likely 100x lower.
None of this means microplastics are harmless. Last month's data on brain accumulation still stands. But the numbers driving the panic may have been measuring the scientists, not the environment.
Science catching its own errors is exactly how it's supposed to work.
