Sometimes it's really difficult to tell seizures and syncope apart... m.youtube.com/watch?v=DOHGXo… (Amazing video of med students making themselves faint)

No - should be a ladder, you may need to reach for a more advanced technique straight away - by using a ‘step wise’ approach you may miss your opportunity imho #MedChat

Stepwise approach is correct, but in my humble opinion it needs to be in the opposite order for pt’s who have no airway self control. ET first, if that doesn’t work SGA/LMA etc, if that doesn’t work NP/OP #MedChat

We have @GNairambulance and @BASICSNW to assist but if it’s an airway problem then I would think you may have limited time before it becomes a bigger problem #MedChat

Wow, so many factors to consider. Location of incident, location of nearest AP, location of HEMS Dr, is it at night (for HEMS availability) etc etc etc #MedChat

I’ve asked for para back up, although not for airway on this occasion, and been told nearest is 56 mins away. #MedChat

Obviously varies trust to trust, but I think the airway management tools we have are fine. Better access to training or keeping competences up is probably the biggest thing I would want to change. #MedChat

How about bougies stored in an appropriate way rather than folded up into a bag? iGels over LMA’s would be good in my trust, decent training for ECA’s/Techs into how to assist. Then regular access for paramedics to practice their skills

We should always be working to improve what we have, whether that is developing new tools or enhancing existing. If we didn’t then health care would never improve #progress #MedChat
