The vibes right now. Glad there was a statement against the use of routine anaerobic coverage for aspiration pneumonia; the actual incidence of anaerobes playing a pathogenic role is much lower than previously estimated. #FOAMed #infectiousdiseases

My answer to the question: "What did you do in pharmacy residency?"😅
Honestly, EVERY ICU should have a critical care pharmacist. One of the the major barriers is quantifying our impact to justify the cost of hiring one. Shoutout to all the healthcare professionals who continue to advocate for our us 🙏
🙏 thanks to all the pharmacists out there quietly saving lives. ncbi.nlm.nih.gov/pubmed/3113…

How do you dose therapeutic enoxaparin in obesity? Patient has a BMI of 60 kg/m^2 (166 kg) and is being started on a warfarin bridge with enoxaparin for an acute DVT. I typically recommend dose-adjusting to 0.8 mg/kg Q12H with BMI > 40, with Anti-Xa monitoring.

Really interesting take. Although it may not make a clinically significant difference with the dose limits recommended, keep in mind that the injectable forms have propylene glycol. Shameless plug: don't forget to utilize your pharmacist for therapeutic drug monitoring.
