#echofirst ✅ Example of structural valve degeneration (SVD) in bioprosthetic AV (TEE) ✅ SVD leads to intrinsic permanent changes in bioprosthetic valve (Ca+ deposits & wear and tear, usually after 10-15 years) ✅ More common in younger pts ✅ In this example, excessive movement of calcified cusp suggests leaflet tear & prolapse (DD: endocarditis) ✅ Comments on valve function? ✅ Ref: Pibarot P et al, JACC 2022;80(5):545-561 @iamritu @argulian @sudarshanballa @alex1708ander @NMerke @echo_batman @DavidWienerMD @BryanTanMD @PWesslyMD @HeartOTXHeartMD @purviparwani

A great question! Left heart filling is not affected too much by respiration except when there is a condition for pulsus paradoxus such as constriction, COPD, obesity, etc. Even in those conditions, PV flow decreases with inspiration, but I am not sure how much S/D changes.
Replying to @JaeKOh2
can S/D ratio of PV change during respiration? and if so should we measure at end expiration?

From 3% to 40% five-year cardiovascular risk and BP lowering across every single decile... At ESC Congress 2026, Prof Kazem Rahimi presented findings from a meta-analysis of ~350,000 participants across 51 randomised trials. Using a cardiovascular risk prediction model, the population was divided into 10 risk groups, spanning from near-healthy patients with a 3% five-year event risk, to very high-risk individuals with a 40% probability of a major cardiovascular event within five years. The landmark result: a 5 mmHg reduction in systolic blood pressure produced a remarkably consistent ~10% relative risk reduction in major cardiovascular events across all 10 deciles. The full interview with Prof Rahimi, hosted by Dr Harriette Van Spall, is now available radc.life/WPd9I0ra #ESCCongress2026 #MedEd #Cardiology

Can we lower risk after MI by lowering LDL-C earlier? 🎙️ At #ESCCongress2026, Dr Harriette Van Spall speaks with Prof Gilles Montalescot about #AMUNDSEN, a trial evaluating evolocumab before PCI in STEMI and NSTEMI patients. Excessive left ventricular trabeculation (#LVET), traditionally viewed as the hallmark of left ventricular non-compaction (#LVNC), is increasingly recognized as a heterogeneous phenotypic trait rather than a distinct cardiomyopathy. This review examines the evolving evidence on its prevalence, imaging characteristics, genetic architecture, and clinical significance across both physiological and pathological conditions. We discuss the mechanisms underlying excessive trabeculation, the potential for overdiagnosis, and whether LVET represents adaptive remodeling, a marker of shared cardiomyopathic substrates, or a phenotypic modifier influencing outcomes (link.springer.com/article/10…). #HREV

How should palliative care fit into advanced HF care? The HeartPal program embedded palliative care within an advanced heart disease service, resulting in earlier referrals, more GoC discussions, fewer late referrals & reduced in-hospital deaths. bit.ly/3S4btlS

Although the health-related benefits of higher fitness and exercise training in both healthy individuals and patients with major comorbidities are widely recognized, participation in regular exercise and exercise training (ET) remains underutilized, especially in high-risk individuals. professional.heart.org/en/sc… Certain high-risk populations, such as those with frailty, stroke, spinal cord injury, rheumatologic conditions, genetic cardiomyopathies, or recipients of advanced heart failure therapies or cardiac implantable electronic devices, warrant special considerations with regard to ET. ✍🏼 @JRGolbus @FrailtyMD 📷 Supporting exercise among high-risk populations.
