How did a $1 billion drug come out of my lab?
I was a basic scientist with zero background in clinical trials, a fledgling assistant professor at @washumedicine. My secret: I asked stupid questions and ignored the conventional wisdom.
Around 2014, we realized JAK1 is expressed in sensory neurons. When we deleted JAK1 in itch nerves, the mice stopped scratching. We predicted JAK1 inhibition would be a major drug target for itch in @CellCellPress. We even got a patent.
@Incyte already had Jakafi (ruxolitinib) approved for myeloproliferative disorders, so I approached them about oral Jakafi for itch. Their answer: "Design a trial, but can you do it with topical ruxolitinib?"
So I designed the pivotal phase 2 trial at a @Starbucks.
What happened next was nothing short of amazing. Incyte had no dermatology division at the time, which meant nobody told me what I couldn't do. I designed the boldest trial imaginable: ruxolitinib head-to-head against triamcinolone 0.1%, the gold standard medium-potency steroid. No dermatology company in its right mind would take that risk.
When the trial read out, I literally celebrated. Topical ruxolitinib showed incredible efficacy across all doses, and it beat triamcinolone on itch. That led to a phase 3 trial and then approval.
Opzelura was approved in 2021, and Incyte's 2026 guidance projects roughly $1.05 to $1.1 billion in net sales. Topical steroids are falling out of favor.
The drug is amazing. Science matters.
Our goal is to do this over and over again @NorthwellHealth.

Why it matters: falling NAD and rising inflammation are two well-known features of aging. Inflammules point to one structure that could link them, and give scientists something specific to test.
Huge credit to Minyan Zheng, who has studied NAD and inflammation for years.

Visualize genomic data with ease using gggenomes, an R package that extends ggplot2 to handle and display genomic information intuitively. Whether you’re comparing genomes, analyzing features, or showcasing synteny, gggenomes provides the tools you need to turn complex genomic data into clear, informative visualizations.
Why use gggenomes?
✔️ Genomic-focused visualizations: Specifically designed for handling genomic data, including features, alignments, and comparative analysis.
✔️ Versatile and modular: Create detailed and layered plots for diverse genomic scenarios with flexibility for customization.
✔️ Built on ggplot2: Leverages ggplot2’s familiar framework, making it easy for users to adapt and enhance their visualizations.
The example visualization shown here is taken directly from the gggenomes GitHub repository, demonstrating how it transforms genomic data into compelling plots: github.com/thackl/gggenomes

What happens to the gut when we profoundly eliminate the B-cell compartment?
CAR-T therapy has transformed multiple myeloma, but as BCMA-directed therapies expand, we are beginning to see consequences beyond the tumor.
A new study characterizes CAR-T cell-induced enterocolitis (CAR-TEC) after cilta-cel and provides a striking look at what may be happening within the intestinal mucosa.
Using single-cell transcriptomics, flow cytometry, and tissue imaging of intestinal biopsies, investigators compared patients with CAR-TEC, CAR-T–treated patients without enterocolitis, and healthy volunteers.
They found a remarkably disrupted mucosal ecosystem:
-Profound depletion of mucosal B cells and plasma cells
-Expansion of highly cytotoxic CAR-T cells
-Inflammatory remodeling across myeloid, stromal, and glial compartments
-Telocyte-associated stromal niche dysfunction
And importantly, interferon/JAK-STAT–associated reprogramming was detected across stromal, endothelial, and epithelial compartments.
That observation provided a mechanistic rationale for targeting the pathway.
In two patients, treatment with the selective JAK1 inhibitor upadacitinib was associated with clinical, endoscopic, and histologic improvement.
The bigger story is that CAR-TEC may not simply represent “GI toxicity” from CAR-T.
It may represent a multicellular failure of mucosal homeostasis arising from profound B-cell/plasma-cell depletion, persistent immune activation, and remodeling of the intestinal tissue environment.
As B-cell–targeted therapies expand beyond oncology and into autoimmune disease, understanding what happens to the tissues that depend on these immune populations may become increasingly important.
The next frontier of CAR-T safety may be understanding not only what happens to the tumor—but what happens to the immune ecosystems surrounding it.

I've attempted to map everything in oncology in a public website and open source repo for all.
The site is trying to get all cancers, products, technologies, bottlenecks, people, startup opportunities with 1000+ ideas for upgrading the field.
If you have a loved one with cancer and you are technical or can engineer, go take a look, file improvement requests or bugs or help with the open repo and make this the best open and free info resource for individuals, researchers and educational use.
This should save people time, aid AI oncology projects and generate positive action. If you are not technical just complain in this thread about broken or annoying or things you want and I'll fix them live.
Some of the interesting pages:

If you are biologically/medically minded and something is wrong, file a bug as well or say on the thread and we will get it fixed live.
If this is a useful project star the repo and help get it calibrated.
I've tried to add some other languages but I cannot speak them so tell me if that doesnt work well.
I believe we will crack oncology and having total information dominance is key to the problem.
Let the feedback flow!
