For anyone interested in how to create a care plan that is truly centred on the individual, read our good practice guide.
It was written for nurses, medical and care staff, after a wide consultation.

How to change suicide risk management in clinical care after recent @NCISH_UK studies?
•routine enquiry re online risk, dom violence, anniversaries
•enquiry about opiates in bathroom cabinet
•stop using risk ass tools
•stop excluding suicidal patients on grounds of capacity.
Margaret MM Conlon retweeted
In our new report @NCISH_UK, we show suicides by patients diagnosed with personality disorder to be rising.
Many have multiple risk factors but downgrading of risk persists because of spurious beliefs re capacity, impulsiveness, “behaviour”.
These ideas are costing lives.
