An intriguing 15-year follow-up of patients with isolated generalized dystonia treated with bilateral GPi-DBS.
The therapeutic trajectories diverged considerably over time, with only 41% maintaining meaningful benefit. Strikingly, 94% developed parkinsonian signs, predominantly axial, and 67% reported freezing of gait.

Apathy is one of the most prevalent yet underrecognized non-motor symptoms of Lewy body disorders. It affects motivation, reward processing, emotion, executive function, and self-initiation—and is associated with faster cognitive decline, greater functional dependence, poorer quality of life, and increased caregiver burden.

Are there really meaningful differences between α-Syn-SAA–negative and –positive Parkinson’s disease?
Brooker and colleagues report that, apart from greater hyposmia in the SAA+ group and more subcortical atrophy in the SAA− group, the phenotypes appear remarkably similar.

Does cannabis-based oil improve pain in Parkinson’s disease?
A phase II, randomized, placebo-controlled trial by Kubota et al. found no significant improvement in pain measured by the PD-Pain Classification System with a CBD/THC formulation (CBD 96 mg/mL; THC 2.1 mg/mL). loom.ly/VYh5T5M
In this important study, repeat MRI-guided focused ultrasound thalamotomy restored meaningful tremor control in patients with ET and tremor-predominant PD. Although repeat treatment was associated with more side effects, 85% of patients would choose retreatment again. New data from a multicenter longitudinal PAF cohort: MDS prodromal MSA criteria identified nearly all future MSA cases (92–100% sensitivity), but specificity was modest. Patients who phenoconverted to MSA were younger and had more severe urinary dysfunction and greater orthostatic heart rate responses. In this interesting study, Puisieux and colleagues report significantly higher plasma homocysteine levels in both symptomatic and premanifest Huntington's disease compared with controls.
