Beautiful example of two tumors in one : papillary RCC and acquired cystic disease–associated RCC forming a collision tumor. One component shows tubulopapillary architecture (separate tumor 1.8 cm) with diffuse CK7 and AMACR expression. The second shows both sieve-like microcysts, eosinophilic cells and calcium oxalate crystals; patchy CK7 Background kidney: End stage renal disease with cysts, #DxMindMap #KidneyPath #GUPath #PathTwitter

Rare UC subtypes, two largest series to date, one message: morphology still predicts behavior. 🔬 Giant cell UC-nuclei ~12x larger than neighboring tumor cells, enriched for T3/4, worse OS PMID: 42252958 🔬 Lipid-rich UC-MDM2(+) could be a pitfall PMID: 42596534 @AJSPJournal

WHAT'S NEW IN NEUROENDOCRINE TUMORS (WHO 6TH EDITION 2026) Presence of any nuclear labeling with Ki67 above background is considered positive Ki67 stain is not a positive or negative stain. It is dynamically expressed throughout the cell cycle Stronger Ki67 IHC assays tend to flatten this variation Dr. Bellizzi GIPS pathcast youtube.com/live/o8IqIDiN_Jw… #pathology #PathX

WHAT'S NEW IN APPENDICEAL TUMORS (WHO 6TH EDITION) Both appendiceal mucinous adenoma and serrated polyp can be circumferential with intact muscularis mucosae • Mucinous adenoma → tall mucinous cells, villous/flat • Serrated polyp → serrated crypts Loss of MM or mural fibrosis = LAMN WHO 6th brings mucinous adenoma back; it was not a distinct category in the 5th edition Dr. Pai GIPSPathcast youtube.com/live/sqSRogzCDf8… #pathology #PathX

Prostatic stromal tumor of uncertain malignant potential (STUMP). Variable morphology: This one show atypical stromal cells, some with degenerative change, proliferating around crowded, architecturally complex benign glands. Common pitfall: BPH stromal nodules are bland and multinodular, with prominent thick-walled vessels. Hypercellularity, marked atypia, increased mitotic activity, atypical mitoses, necrosis, or invasion favor prostatic stromal sarcoma; exclude other sarcomas and sarcomatoid carcinoma. 5 Things Every Pathology Resident Should Know About Invasive Breast Carcinoma NST 2. The Nottingham grade Histologic grade is determined using the Nottingham grading system, developed by renowned pathologists Drs. Ellis and Elston at the University of Nottingham in the UK. It scores three features: - Tubule formation - Nuclear pleomorphism - Mitotic count The three scores are added: 3–5 = Grade 1 6–7 = Grade 2 8–9 = Grade 3 More than three decades after its introduction, the Nottingham grading system remains one of the most important prognostic tools in breast pathology. #PathX #PathTwitter #breastpath

Lung - Lepidic pattern and micropapillary pattern can show as a ground glass lesion on imaging That is why our job is to recognize the micropapillary pattern which is commonly missed (considered high grade, and any tumor with >20% of high grade component is considered poorly differentiated) Even 5% of micropapillary or solid pattern is associated with worse outcomes Classic micropapillary pattern (Pic 1) Not so classic presentations of micropapillary pattern (pic 2) Dr. Butt #JobPrepBootcamp #NIPCAP #pathology #PathX
