A holy GRAIL for MCED and cancer diagnostics.
Q1 Safety. 10-0
Q2 Efficacy. 6-4
Q3 Benefit-risk. 7-2-1
There will be enormous discussions and debate over the next months re operationalization for both individuals (pts, docs, PCPs, surgeons, oncologists) and populations (hospital systems, professional orgs, reimbursements).
Hoping all stakeholders come to the table with the same care this AdComm panel showed today.

Durvalumab + BCG is FDA approved in high risk NMIBC , by hitting its DFS - HR 0.68 (plus a ~⬇️ cystectomy rate). OS & M1 data show these patients have low cancer mortality & IO has about a 10% chance of serious side effects. Therefore, this is not a treatment for all HR NMIBC IMO. It’s hard to know who to select. Inconsistency with other data for Sasanlimab + atezo means there is no subset that consistently benefits. It will be interesting to see whom urologists recommend for treatment. #ASCO26 astrazeneca.com/content/astr…

RevMed with huge news for pancreatic cancer (daraxonrasib).
The headline is hard to ignore 👀
• Median OS 13.2 vs 6.7 months
• HR ~0.4 📉
• Activity across KRAS variants (not just G12C)
• Manageable safety
If that signal is real, this isn’t incremental. It changes how we think about KRAS.
But—caveat applies ⚠️
This is a press release! PR as a rule is rosier than real data.
Still… an OS HR like that gets your attention.
Feels like we may be moving from mutation-specific inhibition → broader KRAS control.
That’s the difference between niche utility and something that touches CRC, pancreas, lung… everything we see.
Now we wait for the data behind the headline.

Among men with locally advanced prostate cancer, transdermal estradiol was noninferior to LHRH agonists for 3-year metastasis-free survival and led to a lower incidence of hot flashes but a higher incidence of gynecomastia. Going to be difficult to recommend a tx for possible recurrence in adjuvant setting with now ⬆️ 50% G3 toxicity without OS benefit . The uptake of pembro alone in this setting has been poor👎 does this make it really any better ? Benefit for few but toxicity for all
Adjuvant Pembrolizumab plus Belzutifan in RCC . Litespark022 . Impressive DFS curves. It will become new SOC if it gets OS . Looking forward to OS data in future @DrChoueiri @tompowles1 @OncBrothers @OncoAlert @ASCO #GU26

Clinical and molecular criteria for aggressive variant #ProstateCancer. @aaparicioMD @MDAndersonNews joins Oliver Sartor, MD @EJHospital to discuss how AVPC captures androgen‑indifferent tumors with combined tumor suppressor defects, early data from chemo + anti–PD‑1 followed by PARP inhibitor maintenance showing benefit in a subset, and the urgent need for better predictive biomarkers. Dr. Aparicio calls for platform trials to test rational combinations and match therapies to specific biological subsets in this hard‑to‑treat disease. #WatchNow > bit.ly/3O9Hze8 @myESMO

When I started out, I thought my job would mostly be clinic and clinical trials. Take care of patients. Design studies. Move the needle a little at a time.
Fast forward a few years and somehow that has turned into running trials, building enterprise AI tools, obsessing over biomarkers, arguing about liver-directed therapy… and still sitting in clinic every week having very real, very human conversations.
Somewhere along the way it started to feel like there was more to say.
GI oncology is moving fast. Data drops weekly. Technology is creeping into everything. We’re learning in real time. And a lot of the nuance never makes it into a manuscript. It definitely doesn’t fit in a tweet.
We wanted a place to unpack that.
So we made one.
The Gut Onc Lab 🎙️
