Here are some facts about money in healthcare that most people in congress, and many doctors, don't realize- 1. Doctors don't get a tax write-off for charity care or unpaid bills 2. Medicare hasn't increased payment rates to doctors since 1992 3. Seeing a hospital-employed doctor costs twice as much as seeing a private doctor in the same office building, because of facility fees 4. We spend $50-60 Billion per year in facility fees for office visits, enough to fund ACA subsidies and finally tie physician payments to inflation, and still have $ leftover 5. After overhead and taxes, a surgeon takes home about $300 for doing a complex 3-hour revision hip replacement 6. Doctor and nurse salaries account for a smaller portion of Healthcare spending in America than in our peer countries (Combined 16%) 7. Total Administration spending in America is 30% of Healthcare costs, which is substantially higher than any other country (Singapore is 3-5%) 8. Healthcare administrator salaries have tripled since 1992, while physician payments per work-RVU from Medicare have increased 3% Got any more to add?

Doctors are paid too much… @linemonkeymd @PhysStaffSolns @DutchRojas @mcuban @DrDiGiorgio @drdanchoi Only 2 parties bring money/value into care: physicians and patients. Everything else is an INTENTIONAL distraction to leech money. Don’t lose sight of this absolute truth.

Medicare Payments to Doctors Have Dropped, Even Before Inflation According to the American Medical Association’s own records, Medicare pays doctors less today in raw dollars than it did in 1998. 1998 Medicare Conversion Factor: $36.69 2024 Medicare Conversion Factor: $33.27 That’s about a 9% cut in nominal terms (before even considering inflation). Inflation Makes It Even Worse The cost of living has nearly doubled since 1998: 1998 Consumer Price Index (CPI): 163.0 2025 CPI: 323.0 Total inflation: +98% That means $1 in 1998 buys almost twice as much as $1 today. When you adjust Medicare payments for inflation, the picture is brutal: Nominal payments are down ~9%. After inflation, doctor pay under Medicare has effectively fallen by about one-third since 2001 (AMA analysis). Why This Matters Every year, doctors face rising costs, rent, staff salaries, malpractice insurance, medical supplies, all driven higher by inflation. But Medicare, bought and paid for by health systems and carriers, payments haven’t just failed to keep up; they’ve moved backward. This isn’t an abstract accounting problem. It’s one of the key reasons independent practices sell to PE and Health Systems, physicians leave medicine, and patients struggle to find care. And Congress could fix it. But that would require physicians to pay them $1B instead of insurance and health systems.

Today I rounded in the hospital on my patients and saw many physicians wandering around away from their family along with dedicated nurses, security, sanitation team, and others. One doctor had a list of 5 hospitals as he checked off patients. Healthcare is a 24 hour a day, 7 day a week job. Many physicians don’t take time off. Thank you to our dedicated healthcare professionals and those who support us!

Boeing failed because business people dictated how engineers should build airplanes Legacy media failed because business people dictated how journalists should report the news Healthcare in 🇺🇸 is failing because business people are dictating how physicians should practice medicine Limit the influence of business people to allow the professionals to do what they do best Bigger is not better. Corporations in medicine like UHC, Aetna/CVS and big health systems that are the biggest employers in every metro area are strangulating competition and driving up prices. Level the playing field to restore the patient physician relationship that all patients crave and deserve. Break up the horizontal and vertical mergers in medicine that are destroying independent practices and holding physicians as captive employees! cc: @DOGE @VivekGRamaswamy @elonmusk @RobertKennedyJr @DavidSacks @CaseyMeansMD @Jason @chamath @DrOz

I see patients from all socioeconomic strata Many of them come to my oncology clinic to confront their own mortality No matter how much money you have, the ultimate currency is time Which is why it makes me so angry when insurers INTENTIONALLY waste the most precious resource

Screening is important, and even more important is #hpv vaccination, let's prevent the 36,000 HPV related cancers that patients will be diagnosed with this year in the US. The HPV vaccine has been available since 2006, with 135 million doses given. It's #cervicalcancer awareness month, we should do better than the 60% vaccination rate in the US. HPV related cervical cancer is preventable. @SGO_org
#Cervicalcancer screening may involve Pap tests, HPV testing, or both. For each type, cells are taken from the cervix and sent to a lab for testing. View our full infographic to determine when to get this screening: bit.ly/3tInnGi #CervicalHealthMonth

Physicians keep seeing our Medicare rates slashed (down 20% since 2000). Yet, the Medicare program is still facing insolvency. Medicaid spending is the largest share of state budgets. Both programs continue to grow at a faster pace than the overall economy. Can spending be curbed without further harming patients? Yes. In an excellent @Health_Affairs piece, Deborah Williams, @DrBrian4Health, @MedicareMeddler, @betty_rambur and @gailwilenskycom outline pragmatic, realistic steps to reduce costs without hurting patients. 1- Site neutral payments: Why does a hospital clinic get to charge more for the same office visit as an independent practice. Fix this loophole. 2- Reform GME funding: The money that hospitals get in exchange for training residents is not commensurate with the costs. Hospitals are expanding residency spots without GME funding, so there's clearly a market force at work. 3- Equalize tax treatments for investments: 93% of hospital holdings are unrelated to their core mission. Taxing these gains could provide needed funding for a variety of programs. 4- Fix state funding of federal funding schemes: Since the federal government reimburses states for Medicaid spending, states use a variety of accounting tricks, such as provider taxes and 340B, to get more funding. This is essentially robbing Peter to pay Paul. Policy is complex, but physicians must understand these issues so we can advocate for financially responsible, patient-centered care. Neurosurgery training is nothing compared to this! healthaffairs.org/content/fo… @ucsf_ihps @cscla @AnilMakam @VPrasadMDMPH @EconTalker @neurosurgery @SpineNeuro @JPGK_MD @JoanneSpetz

If 48% of patients still needed adjuvant radiation after neoadjuvant chemotherapy+surgery, AND the short-term toxicity of this approach still doubles that of CCRT (41% vs 23%), what does NACT add to radical surgery? I guess less (15% vs 21%) long-term toxicity+pt peace of mind?
