More than a decade ago, the trend led by top medical schools was 2-tier (pass/fail) grading for preclinical courses. The story of the last decade is pass/fail for core clinical clerkships (now at 12 of the top 25 schools). As a result, residency programs have more difficulty differentiating students from these schools, and students are increasingly driven to pursue extracurriculars (chiefly research/publications) and max their Step 2 score to distinguish themselves.

Take-aways: For referrers: read the radiology report. For radiologists: close that loop if you think the incidental finding is significant (judgment applies to what that means). Medical educators should help learners recognize when a case requires cross-domain synthesis. Start by making explicit that a complex case may fall in the "interstitial space" between specialties. Then use AI to explore these interstitial spaces. @jennyxchen in @JournalofGME #MedEd doi.org/10.4300/JGME-D-26-00…

🤔Could embolization become a treatment for syringomyelia? 📰A remarkable case: after two failed Chiari decompressions, embolization of a thoracic CSF-venous fistula led to near-complete syrinx resolution. 🚀Early evidence, but an exciting therapeutic possibility! doi.org/10.3171/CASE26202

There’s more information in every CT scan. Opportunistic screening can extract biomarkers such as bone density, body composition and vascular calcification from existing imaging, expanding radiology’s role in proactive patient care. @SidDograMD @NYUImaging bit.ly/4jHPkWf

Check out this month’s #AJNR Fellows’ Journal Club choice: ‘’Sacral Dural Tears in Spontaneous Intracranial Hypotension: Imaging Phenotype and Treatment Outcomes in a Multicenter Cohort’’ Sacral dural tears are an often overlooked cause of spontaneous intracranial hypotension, with treatment outcomes primarily influenced by baseline imaging features and disease chronicity rather than procedural technique In this retrospective multicenter cohort, unorganized extradural CSF collections predicted better clinical and imaging outcomes, while epidural patching achieved complete clinical improvement in 56% of patients. Dynamic CT myelography outperformed digital subtraction myelography in leak localization (75% vs 31%). These findings highlight the importance of morohology based diagnosis and sacral-directed imaging and treatment. ajnr.org/content/47/10/2674 #neuroscience #radiology #radiologyresident #intracranialhypertension #sacralduraltears
