🫀From Healthy Atria to Atrial Failure: Rethinking Atrial Cardiomyopathy (AtCM) from the #HFA_ESC clinical consensus statement:
📍AtCM extends beyond #AF: Electrical, mechanical & structural abnormalities may precede clinically apparent AF or #HeartFailure
📍Diagnosis: Electrical atrial dysfunction (P-wave score ≥1) PLUS ≥1 of: mechanical dysfunction, atrial enlargement or excessive atrial fibrosis
📍Proposed diagnostic thresholds:
- P-wave ≥120 ms (standard ECG) or ≥150 ms (amplified ECG)
- Advanced interatrial block: P-wave ≥120 ms + biphasic morphology in ≥2 inferior leads
- LA reservoir strain <23%
- LAVi >40 mL/m² (>48 mL/m² in women >65 years)
- Atrial fibrosis: thresholds pending validation
📍New P-wave scoring system (0–4): Ranges from normal P-wave (0), prolonged P-wave (1), advanced interatrial block without (2) or with paroxysmal arrhythmia (3), to persistent atrial arrhythmia (4).
📍Atrial failure: End-stage AtCM, characterized by persistent AF or HF with symptoms attributable to atrial disease.
📍Early recognition matters! Risk factor & comorbidity management may help prevent or delay progression to atrial failure.

🫀 Are we measuring pericardial effusions the wrong way?
We routinely classify pericardial effusions as small, moderate, or large based on linear measurements.
But does a single measurement in millimeters accurately represent the actual volume of pericardial fluid?

Median drained volume: 500 mL (range 50–4,300 mL)
76.7% had cardiac tamponade
Actual drained volume served as the reference standard.
📊 How did different echo methods perform?
Between end diastolic thickness, plax and PSAx area- apical 4 chamber Simpson’s volumetric estimation performed the best
✅ Accuracy: 80.4% vs 49.4%
Perhaps it is time to rethink how we quantify pericardial effusions!
💡 if volumetric assessment is substantially more accurate, should we move from millimeters to milliliters in routine echocardiography?
And more importantly, would this change clinical decision-making—or simply improve the precision of our reports?

Si un paciente con FA presenta un ACVi mientras recibe anticoagulación, el riesgo de transformación hemorrágica obliga a suspenderla.
La pregunta de siempre es: ¿cuándo reiniciarla?
Esta gráfica ayuda a responderlo y, en general, favorece el reinicio durante la primera semana especialmente si no existe transformación hemorrágica o no es extensa la isquemia.

Revascularization of #CAD in #HeartFailure: who, when, & how?
This #EJHF Expert Consensus Statement proposes a practical, symptom-based approach:
📍Start with the symptoms: are they predominantly HF-related or angina-related?
📍Optimize GDMT first in all patients with HF, & also assess for eligibility for CRT +/- invasive procedures (eg M-TEER, VAD)
📍Persistent angina despite GDMT? Consider revascularization, with assessment of coronary anatomy, ischaemia & viability where appropriate.
📍CABG or PCI? Consider surgical risk & coronary complexity, with PCI an option in patients at higher surgical risk or with less complex coronary disease.
📍Heart Team discussion is key to individualized decision-making.

Guías 2026 de la AHA de manejo preoperatorio en cirugía no cardíaca (alias valoración preoperatoria). Recién salida del horno ayer. Puntos importantes:
🔴 Los iSGLT2 se suspenden 3 días antes por riesgo de cetoacidosis euglucémica (nota: la evidencia nueva apunta a que tal vez no se requiera).
🔴 Los aGLP-1 semanales se suspenden más de 1 semana antes (por broncoaspiración). Metformina: continuar.
🔴 Betabloqueador: continuar. Si apenas lo vas a iniciar: empezar 7 días antes.
🔴 IECA/ARA-II pueden omitirse 24 horas en hipertensión controlada + cirugía de alto riesgo, pero se continúan en insuficiencia cardíaca.
🔴 Prueba de estrés y angioTC coronaria: solo con riesgo alto (como <4 METs por DASI y otra indicación. CAC de 0 en los últimos 2 años: pasa a cirugía sin más.
🔴 El ECG en asintomáticos con cirugía de bajo riesgo: no aporta, no es necesario.
🔴 En cirugía de alto riesgo, >65 años, o sintomáticos: considera medir BNP o NT-proBNP). En general no se necesitan seriados.
🔴 Anticoagulación oral (con ACOD) sin puente en general, sin heparina. Puente solo si: AVK y riesgo trombótico muy alto: válvula mecánica mitral, EVC/AIT en los últimos 3 meses, trombofilia o CHA₂DS₂-VASc ≥7.
🔴 Tiempos e infarto. Después de un stent farmacoactivo: esperar 6-12 meses si cirugía electiva. Después de un EVC/AIT se esperan ≥3 meses.

⚠️ التوصيات الحديثة لنقص الحديد وضعت حدود واضحة في متى يسمى نقص حديد بغض النظر عن التغير بالهيموجلوبين 🩸
وهذه نقطة جيدة خصوصًا لبدايات المشكلة وكذلك لحل مشكلة التركيز على الهيموجلوبين فقط دون التركيز على مخزون الحديد
💡 لكن لهذا تفصيل فالرقم ليس واحد للجميع
- الاشخاص في حالات الالتهاب، القولون الالتهابي مثل كرونز والقولون التقرحي، الاورام، الأمراض المعدية والالتهابية
يشخص نقص الحديد ان كان اقل من ١٠٠
- بشكل عام للكبار يشخص نقص الحديد ان كان اقل من ٣٠ وللاطفال تحت ٤ سنوات اقل من ٢٠
- ولكن عند وجود دورة شهرية غزيرة، او حمل مع وجود فقر دم يشخص نقص الحديد ان كان اقل من ٥٠
ولابد الانتباه لنقطة (تشخيص نقص) و (رقم غير كاف لزوال الاعراض)
- فلا تزال توصية الخبراء عند بعض الاعراض مثل تساقط الشعر الكربي، او الشقيقة فان الرقم الموصى به (وشوهد تحسن معه) هو اعلى من ٧٠ (قد لا يسمى نقص حديد، لكن يعتبر غير كاف لزوال الاعراض)
🟢 مثال: امرأة تعاني من تساقط كربي مستمر، مخزون حديد ٥٠، لايوجد مسببات اخرى (يفضل رفعه اكثر من ٧٠ ومراقبة التحسن)

كما ان التوصيات تستمر كما هي في الطريقة العلاجية الصحيحة
- يفضل العلاج بالفم (بالحبوب والتغذية)
- العلاج الوريدي لحالات محددة فقط
وبالنهاية، اختر طبيبك بعناية، واستشر قبل اخذ اي علاج

Calcificación Anular Mitral. 🫀❄️💥
💠La CAM es un proceso degenerativo crónico de calcificación del anillo mitral, predominantemente posterior.
💠F. Riesgo: edad avanzada, sexo femenino, ERC, HVI, HTA, estenosis aórtica, radioterapia mediastinal.
💠Diferencia con la EM reumática: en la CAM la calcificación progresa desde el anillo hacia las valvas, restringiendo su movilidad, pero sin fusión comisural.
📐 Clasificación anatómica propuesta:
Grado 1️⃣ (leve): calcificación focal no contigua <180°.
Grado 2️⃣ (moderado): calcificación densa continua <270°.
Grado 3️⃣ (grave): calcificación que sobrepasa las comisuras hacia el anillo anterior o circunferencial ≥270°, pudiendo extenderse a valvas, músculos papilares o miocardio.
💠Tratamiento: no existe terapia médica específica que revierta la MAC y la valvuloplastia mitral con balón no es apropiada para la EM calcificada. La cirugía puede requerir desbridamiento y reconstrucción del anillo, pero en MAC extensa presenta alta morbimortalidad, incluyendo riesgo de disrupción del surco AV. ⚠️🚫
