Anemia en enfermedad renal crónica (ERC), una sinopsis de las guías KDIGO en Annals of Internal Medicine (2026). Puntos clave:
🟠 En HD: iniciar hierro (IV) si ferritina ≤500 ng/mL y TSAT ≤30%, suspender si ferritina >700 o TSAT ≥40%. Sin HD o en DP: ferritina <100 con TSAT <40%, o ferritina 100–300 con TSAT <25%.
🟠 ESA (agente estimulante de la eritropoyesis) primero, HIF-PHI (Hypoxia-Inducible Factor Prolyl Hydroxylase Inhibitors) después. HIF-PHI se evita en cáncer activo, poliquistosis, retinopatía proliferativa, HAP, eventos CV o tromboembólicos previos y embarazo.
🟠 Meta de hemoglobina: <11.5 g/dL, típicamente 10–11.5. Es la única recomendación fuerte de las 8. Base: CHOIR Trial (2006).
🟠 Inicio de ESA individualizado. En G5D: Hb ≤9–10 g/dL. Sin diálisis: 8.5–10 g/dL (debatido). Siempre siempre corrige antes las otras causas de anemia y reponer el hierro.
