A recent study from the Yorkshire Heart Centre demonstrates how targeted CMR protocol redesign can reduce scan times and expand imaging capacity. The study evaluated a “Short Aorta” CMR protocol for patients referred primarily for thoracic aortic assessment. Unlike the standard protocol, the shorter approach focused only on the thoracic aorta when additional assessment of the aortic valve and left ventricular function was not clinically required. Key findings: ✅ 137 referrals were reviewed ✅ 69% were clinically suitable for the shorter protocol ✅ 46% of scans used the “Short Aorta” protocol after implementation ✅ Median scan time decreased from 37 to 18 minutes ✅ Approximately 1.2 hours of scanner time per week was saved ✅ With optimized scheduling, potential savings increased to 1.9 hours per week These findings highlight how thoughtful protocol redesign—not only new technology—can improve CMR workflow, reduce patient waiting times, and increase imaging capacity. The shorter protocol should be used selectively and with appropriate clinical triage. Patients requiring assessment of the aortic valve, left ventricular function, or other cardiac structures should continue to undergo the full CMR protocol. 🔗 Read the full study: doi.org/10.1093/ehjimp/qyag1… @EHJIMPEiC @VikashJaiswalMD @saramoscatelli7 #CardiacMRI #CMR #CardiovascularImaging #AorticDisease #MedicalImaging #HealthcareInnovation #QualityImprovement #Radiology #Cardiology #ImagingEfficiency #PatientCare #CardioX #CardioEd

Important #JACCImaging study of ~7.4K patients with ascending aortopathy comparing TTE with gated CTA. TTE underestimated aortic size in: 🔹 58% of aortic roots 🔹 37% of mid-ascending aortas The discrepancy was particularly important with larger aneurysms. Beyond absolute diameter, height-indexed aortic diameter and cross-sectional area/height may improve risk stratification. ⭐️ When the aorta is getting large, cross-sectional imaging matters. #yesCCT #Echofirst #Aortopathy #CVImaging 🔗 jacc.org/doi/10.1016/j.jcmg.…

🫀 Can a single free-breathing 4D flow CMR scan replace multiple conventional acquisitions? A new study presents a deep learning model that automatically derives whole-heart volumetrics and advanced haemodynamics directly from 4D flow CMR magnitude images, dramatically reducing analysis time while maintaining excellent accuracy. 🔍 Key findings: ✅ 4D flow magnitude images accurately quantified cardiac chamber volumes, showing excellent agreement with conventional cine CMR (LVEDV ρ=0.98, ICC=0.99; RVEDV ρ=0.97, ICC=0.98). ✅ The AI segmentation model achieved a mean Dice similarity coefficient of 0.88, with strong agreement between automated and manual measurements for volumetric and haemodynamic parameters. ✅ Advanced flow metrics—including kinetic energy, vorticity and viscous energy loss—could be derived automatically with excellent correlation to manual analysis. ✅ The AI pipeline reduced post-processing time from around 3–4 hours to approximately 2 minutes, paving the way for rapid clinical implementation. 💡 A major step towards a "one-stop-shop" CMR examination, where a single free-breathing 4D flow acquisition could provide comprehensive assessment of cardiac anatomy, function, flow, and energetics with minimal user interaction. 📖 Read the full article: doi.org/10.1093/ehjimp/qyag1… #CardioX #CMR #4DFlow #CardiacMRI #ArtificialIntelligence #DeepLearning #CardiacImaging #Cardiology #MedTwitter #OpenScience #DigitalHealth

🫀 ESC Heart Failure Guidelines 2026: Cardiac Imaging — Beyond LVEF: a major shift in cardiac imaging 👉 Don’t just measure EF — identify the phenotype & cause of HF. Here are the 🔑 imaging takeaways: 🔹 Echo remains first-line TTE remains the cornerstone of HF assessment, providing information on: • LVEF & LV remodelling • RV function • diastolic function/filling pressures • valvular disease • pulmonary pressures • haemodynamic consequences of HF The message is clear: a HF echo should be much more than an EF measurement. 🔹 CMR: define the myocardial substrate CMR is particularly valuable when echo is inadequate or the aetiology of HF/cardiomyopathy remains uncertain. LGE and parametric mapping can identify: ➡️ scar/fibrosis ➡️ inflammation ➡️ infiltration ➡️ iron overload ➡️ characteristic cardiomyopathy patterns This is particularly relevant in suspected myocarditis, amyloidosis, sarcoidosis & other cardiomyopathies. 🔹 CT/CTCA: answer the coronary/anatomical question CT coronary angiography can help define or exclude CAD in appropriately selected patients. When ischaemia rather than coronary anatomy is the question, stress echo, stress CMR, SPECT or PET may be appropriate. 🔹 Amyloidosis: imaging can establish the diagnosis For suspected ATTR cardiac amyloidosis, evaluation integrates: ☢️ bone-tracer scintigraphy 🧪 serum/urine immunofixation + free light chains 🧲 CMR when appropriate Identifying ATTR-CM is increasingly important given the expanding availability of disease-modifying therapies. 🔹 Cardiac sarcoidosis FDG-PET should be considered when cardiac sarcoidosis is suspected. CMR and PET provide complementary information: 🧲 CMR → myocardial injury/scar ☢️ FDG-PET → active inflammation 🔹 HFpEF does not mean normal function Imaging should look beyond LVEF & assess: ➡️ diastolic dysfunction ➡️ LA remodelling ➡️ LV hypertrophy ➡️ filling pressures ➡️ pulmonary pressures ➡️ RV function When resting investigations are inconclusive, exercise testing &/or invasive haemodynamic assessment may be needed. 🔹 Imaging before CRT/ICD LV function should be reassessed after appropriate foundational therapy when the result will determine device eligibility. For CRT, imaging must be integrated with the electrical phenotype. ⚡ CRT remains Class I, LoE A in appropriately selected symptomatic HFrEF patients with: • LVEF ≤35% • sinus rhythm • LBBB • QRS ≥150 ms • despite optimal foundational therapy 🔹 2ndry MR →imaging guides intervention Echo defines MR severity, mechanism, LV/RV function & suitability for intervention. In appropriately selected symptomatic HFrEF + severe 2ndry MR despite optimized therapy ± CRT, mitral TEER now carries a Class I recommendation to ⬇️HF hospitalization & improve QoL. 🔹 Advanced HF: haemodynamics matter In patients being evaluated for transplantation or durable mechanical circulatory support: ➡️ Right-heart cath— Class I, LoE C This provides critical information on filling pressures, cardiac output and pulmonary vascular haemodynamics. 📌 The 2026 take-home message Cardiac imaging in HF is moving from EF → phenotype &→ haemodynamics→ tissue characterization→ aetiology →targeted treatment 💡 Echo tells us how the heart is functioning. CMR tells us about the myocardium. CT defines anatomy. Nuclear imaging identifies specific disease processes. Haemodynamics completes the picture when non-invasive imaging is insufficient. The key question is no longer simply: “What is the EF?” It is: 👉 “Why does this patient have HF— and will additional imaging change management?”

Our ESC Clinical Consensus Statement on Women’s Heart Centres is published in @EurHeartJ merging 4 associations @EACVIPresident @EAPCIPresident @HFA_President @ACVCPresident Link ➡️ academic.oup.com/eurheartj/a… ESC press release ➡️ escardio.org/news/press/pres… This is the first statement to define what a Women’s Heart Centre actually is: its structure, its team, its referral pathways, and the training standards clinicians need to lead one. The problem: CVD is the #1 killer of women. Yet women face systematic delays in diagnosis and treatment and the conditions most likely to affect them (INOCA, MINOCA, pregnancy-related CVD) remain underdiagnosed everywhere. Our solution: hub-and-spoke Women’s Heart Centres, embedded in existing CV systems, not parallel structures. Advanced diagnostics, multidisciplinary care, research coordination. For the first time we define: ✅ The nucleus clinical team ✅ Adjunct specialties (cardio-obstetrics, oncology, rheumatology, psychology and more) ✅ Core vs advanced competencies ✅ A pathway toward subspecialty certification We also set milestones for national and institutional adoption. 📄 4D-flow CMR meets diastology: can we finally assess E/e′ with MRI? 🔗 DOI: doi.org/10.1093/ehjimp/qyag0… 🫀 Diastolic dysfunction (DD) is a key early marker of heart disease—but CMR has traditionally struggled to assess it. This study introduces a novel solution: 👉 CMR-derived 4D-flow E/e′ for biventricular diastolic function assessment ✨ What’s new? 👉 Combines: ✔ 4D-flow transvalvular velocity (E) ✔ CMR-derived tissue velocity (e′) ➡️ Recreating the classic echo parameter (E/e′) using MRI 📊 As shown in the graphical abstract (page 2): 👉 simultaneous evaluation of LV + RV diastolic function ✨ Study at a glance: 🔹 132 participants 75 healthy controls 57 CVD patients 🔹 Full biventricular assessment with 4D-flow + strain/annular motion ✨ Key findings: 📈 4D-E/e′ is higher in CVD patients ➡️ reflecting impaired relaxation 📊 Excellent diagnostic performance: ➡️ LV DD detection: AUC ~0.90 📌 Best method: 👉 annular velocity–based e′ 📉 Hidden disease uncovered: 👉 Among patients WITHOUT prior diastolic assessment: ➡️ 71% had abnormal LV diastolic function ➡️ 61% had abnormal RV diastolic function ⚠️ A major proportion of DD is currently missed in clinical practice 📊 Strong validation vs echocardiography: ✔ E velocity: r = 0.81 ✔ E/e′: r = 0.77 ➡️ As shown in Figure 3 (page 6): 👉 strong correlation with echo 🧠 Why this matters: 👉 CMR already performed in many patients ➡️ Now it can also provide: ✔ diastolic function assessment ✔ biventricular evaluation ✔ detection of early/subclinical disease ⚡ Key innovation: 👉 First robust demonstration of: integrated flow + tissue metric (4D-E/e′) using CMR ➡️ Overcomes limitations of: ❗ 2D flow ❗ static imaging planes 💡 Clinical take-home message: 👉 CMR is no longer “systolic-only” ✔ 4D-flow enables comprehensive haemodynamic assessment ✔ Can detect undiagnosed diastolic dysfunction 🚨 Bottom line: 4D-flow CMR brings echocardiographic E/e′ into MRI—unlocking a new era of full functional cardiac assessment. #Cardiology #CMR #4DFlow #DiastolicDysfunction #HeartFailure #CardiacImaging #EoverEprime #PrecisionMedicine #DigitalHealth 🫀📊
