Today I learned that sometimes hepatocellular carcinoma can be diagnosed on imaging alone. If the patient has cirrhosis, chronic hep B or prior HCC an LI-RADS score of 5 is diagnostic! LI-RADS 4 requires biopsy and LI-RADS 3 or less is likely benign and can be monitored.

Does mental stress affect our mitochondria?
In this new paper, we asked if 5 minutes of mental stress (public speaking) changed the amount of cell-free mitochondrial DNA (cf-mtDNA)
Absolute saliva cf-mtDNA levels increased on average by 280% after mental stress!

ERJOR: A walk test of 3 min still detects most desaturation events with equivalent prognostic value. Shorter walk tests could be used clinically, which would be a more efficient and patient-centred approach and also help the move towards remote monitoring. bit.ly/43rebnv

Everyone in CTD-ILD world, please see this paper on Abatacept for the treatment of myositis-associated interstitial lung disease (ATtackMy-ILD), a RCT in IIM-ILD. Even though primary outcome was not met, but PFTs and HRCT trends before and after Abatacept are interesting and worth considering further larger study. Serum IgG from pulmonary arterial hypertension (PAH) patients show specific reactivities against Annexin A5 (ANXA5) and other antigens not recognized by serum IgG from those without PAH. Anti-Annexin A5 (ANXA5) IgG autoantibodies are detected in one-third of systemic lupus erythematosus (SLE)-associated PAH patients and over two-thirds of mixed connective tissue disease (MCTD)-PAH patients. These findings indicate that anti-ANXA5 IgG antibody reactivity in patients with SLE and MCTD could potentially predict PAH occurrence during follow-up.
