As an outpatient physician, this is the most misunderstood conflict between patients and doctors. From the patient perspective, it’s inconsiderate, especially when many offices will reschedule if you are more than 10 minutes late. Traffic, child care, finding a ride, time off work. So many factors go into just getting to a doctor’s appointment on time. From the physician perspective, there is a giant machine behind the doctor dictating how we run our clinics. Administration, reimbursement, regulatory hurdles like prior authorizations, balancing wait times with patient demand. If you asked most physicians how many patients they would like to see in a vacuum, most would choose a number that is far fewer than their current volume, but the healthcare system has other ideas. I wish we had an empathy contraption. Two people wear it and you instantly understand the other person’s experience. In lieu of that, these types of complaints and disagreements can be remedied with better communication. Give patients updates on the status of the clinic. Vast majority of people are reasonable, just communicate. Have a system for it.
Doctor’s offices be like: “please arrive 15 minutes early so the doctor can see you 45 minutes late.

I support the death penalty when there is no doubt about the intent and guilt of the perpetrator - but the medical community should not be involved. Physicians or medicine should not be instruments of death. Participating in an execution stands against the Hippocratic oath.
🚨 Christa Pike is still alive after lethal injection being overseen by Dr. Mark Fowler. Tennessee used Dr. Fowler in May for Tony Carruthers execution. Fowler and the team botched it. The execution was called off. Carruthers got a 1 year reprieve. Investigate Fowler

Insurance claims denial letters are starting to sound like break-up texts: It's not you, it's us. We just don't find this treatment medically necessary right now. (TBH it's usually more like... It's not you and it's not us... It's your doctor. It's all her fault.)

Ai-Generated content is really annoying. 2 and 3 word sentences quietly Not ___, but ____ Vibes overuse of adjectives and adverbs It's really easy to spot, and it's not good writing. The irony is that some champion AI as "efficient prose." If so, I don't like efficient prose. I like Welty and even Grisham.... but don't give me AI to read seriously.

Most people cannot name even 1 past Secretary of HHS. That's the problem.... RFK Jr. is a consequence.... not the root cause. :)

Want to know why Costplusdrugs.com doesn't have more brands? With a few exceptions (already has a generic, not on formularies, soon to have a generic), CPD , and every pass through smaller PBM , can't buy branded meds that have a rebate, at a competitive price. No one is asking for rebates equal to the big 3 PBMs. We can't even get a " walk in off the street price." We actually had one brand say in a meeting they would give us a "walk in off the street price", then all the sudden they couldn't I asked the CEO of a best selling branded med how many lives we would need to have covered before they would sell to the GPO we created, or to us in any manner, he said "half". They couldn't sell to us until we covered half the lives the big 3 cover. He would lose placement on formulary tiers merely for offering us rebates. He couldn't afford to do business with us This is not an efficient market. The big PBMs have threatened brands with loss of formulary positioning for their entire portfolios This is why brand pricing continues to be high for employers @PamBondi @AFergusonFTC

100%. How do we figure out why we’re paying too much if we can’t see where the money is going?
Replying to @mass_marion
To your point... We need FULL price transparency across the whole system so that the public can see how bad the rip-off really is.... From the Insurer to the PBM to the wholesaler... and yes, the Hospitals. This includes the "proprietary" rebate arrangements. I believe this administration will push this issue in the right direction. But who knows!

The rest of the house of medicine should follow suit. Transitioning children and teens who do not have capacity to give informed consent is immoral and unethical. Many (not all) in the medical community have attempted to pass off Ideology as medicine and it didn’t work… This has resulted in confusion and distrust. The way back is to recant these decisions and stick to evidence… not ideology. I do applaud the plastic surgeons. There’s more work to be done in this area.
The American Society of Plastic Surgeons has recommended against gender transition surgeries for youths, becoming the first major medical association in the U.S. to narrow its guidance on pediatric gender care. wapo.st/3ZiJSxJ

Doctors can fix the system if the handcuffs are removed, the government gets out of the way, and we are allowed to tailor the system to the patients. We don't need nationwide "systems" as a backbone. Nationwide "systems" can be there to support the frontline. The current system is just a grifting mechanism. You know what a "new government program" really is? A revenue stream, pure and simple. Let the Indy Docs and the Indy Pharmacists get back in the game and take care of their piece of the world. You'd be amazed at how much better things get.
This completely ignores the legislative handcuffs placed on doctors. You ask why independent physicians don't 'scale' to access the same funding mechanisms hospitals use. The answer is simple: it’s illegal. Between the ACA's ban on Physician-Owned Hospitals (POH) and the Stark Law, doctors are expressly barred from scaling. If they try to build their own facilities—the ORs and imaging suites you mention—they are literally cut off from the Medicare/Medicaid markets or buried in compliance costs. They also don't have access to the tax-free municipal bonds hospitals use for cheap capital. There’s nothing 'special' about HCA or PE-backed groups. The only thing 'special' about non-profit systems is that they can grow at scale without paying taxes and self-refer across their entire network—something the law calls 'criminal' if a small group of doctors tries to do it with a local ASC. Independent doctors 'ration' Medicaid because they are structurally excluded from the revenue streams and 'State Directed Payments' that hospital lobbyists have secured for themselves. Look at the double standard: When a physician-led facility like Doctors Hospital at Renaissance in McAllen serves a massive Medicaid population, they get labeled 'greedy' by Atul Gawande and the Obama admin. But when a giant 'non-profit' system throttles Medicaid appointments, consolidates to jack up prices, and guts the local economy, we’re told they’re doing 'charitable work.' Finally, let's talk about the GME 'subsidy.' Hospitals get a massive ROI on resident labor. When UNM’s neurosurgery program lost its 10 residents, they had to hire 23 APPs just to cover the volume. Residents 'pay' for their education with 80+ hour weeks at sub-market rates. The hospital isn’t gifting an education, it’s performing labor arbitrage on the most highly skilled workers in the country. Doctors aren't 'choosing' to be passive beneficiaries. They’ve been legally sequestered from the capital and scale required to compete

The answer is at least two-tiered. For those of us who are healthy, the monthly premiums are a scam. We never get our money's worth....never. We are paying for more than peace of mind at the current rates. For those unfortunate ones who have health problems... the Deductibles are the larger burden. I see this daily in my practice.

Want another reason why healthcare costs are insane ? Hospitals will not only charge a facility fee and other random costs BUT ALSO , if they believe the insurance company is willing to pay MORE THAN WHAT WAS ON THE PATIENT BILL, THEY WILL INCREASE THE BILL to the insurance company. Of course the insurance company then charges the self insured employer the higher amount. Our healthcare has become a game of who can rip off who and get away with it. Too Big To Care - all of it. Break em up !
