Who is Enlivex?
People today are living longer than any previous generation, but many of those extra years are spent in poor health.
Globally, the average gap between lifespan and healthspan has now reached 9.6 years. This is what Enlivex was built to address.
The Story:
Enlivex is a quality-longevity company developing Allocetra, a first-in-class immunotherapy for knee osteoarthritis, a condition that affects more than 370 million people worldwide.
Osteoarthritis has no approved therapy that modifies its course, and for roughly a million patients each year in the United States, it ends in joint replacement.
That is where the clinical work started. It is also where a broader question came into focus.
If quality longevity is our mission, how do you build a company that can actually pursue it through the long timelines and capital demands of clinical development?
The Structure:
Our mission is to help people hold onto the mobility, independence, and vitality that make later life worth living.
To power that mission, we built something no other Nasdaq-listed clinical company has built. A corporate treasury anchored to prediction markets, one of the fastest-growing sectors in modern finance.
The treasury holds a significant position in RAIN, the native asset of @Rain__Protocol, giving Enlivex a capital foundation independent of the funding cycles that traditionally define biotech.
Two strategies. One mission.
The quality longevity company built around a prediction markets treasury.
Nasdaq: ENLV.

Before @Enlivex I spent the better part of a decade on a single molecule. A long-acting growth hormone that started as early science and eventually became a therapy Pfizer took to approval in 43 countries.
What that taught me is how long the distance is between a good result in the lab and a drug a patient can actually get. Most of that distance is not science. It's the discipline to keep a program alive through every stage where it could have died.
I came to Enlivex because I wanted to do that again, with a therapy aimed at a disease far more people live with. Knowing exactly how hard the road is doesn't make me less willing to walk it. It's the reason I know it's worth walking.
Can your career shape your odds of developing osteoarthritis?
A meta-analysis of 71 studies covering nearly one million people found that jobs involving repeated kneeling, squatting, heavy lifting, and prolonged standing significantly increase the risk of knee osteoarthritis.
Construction workers, miners, and floor layers had the highest measured risk. Farmers were separately identified as having the highest risk of hip OA.
Repetitive load on the joint, sustained over decades of work, accelerates the disease.
Our quality longevity mission was developed with these people in mind, and the hundreds of millions who share their diagnosis worldwide.
Each year in the United States, roughly one million people undergo knee or hip replacement.
Osteoarthritis is the primary driver. When the disease progresses far enough that cartilage in a load-bearing joint has been substantially lost, and other options no longer restore function, arthroplasty becomes the standard intervention.
The surgery marks the end of a treatment path built around symptom control. No therapy currently exists that alters the disease driving the joint toward replacement.
Our Allocetra program is being developed to reach patients earlier, targeting the immune dysfunction behind knee osteoarthritis before it advances to the point where replacement becomes the only option.

Osteoarthritis does not discriminate.
Billie Jean King was living with knee osteoarthritis by her early 20s. Bobby Orr's career ended at 30 because of it, and decades later, Andy Murray needed hip resurfacing at 31 to keep playing.
These are among the most decorated athletes in the history of their sports, and the disease reached them anyway.
For every name the public knows, there are millions living with the same condition without a platform to share their story. More than 600 million people worldwide have osteoarthritis today, and none of them has an approved therapy that alters the course of the disease.
Our Allocetra program was built to reach anyone living with knee osteoarthritis.
Our quality longevity mission is advancing through clinical milestones.
Our Phase I/IIa data was presented at EULAR 2026 in London, showing statistically significant and clinically meaningful improvements in knee pain and physical function against placebo, sustained through at least 6 months.
The same data supported the FDA's decision to grant Allocetra RMAT designation for age-related knee osteoarthritis, a designation reserved for therapies with the potential to address serious unmet medical needs.
The Phase IIb trial is now underway, enrolling 182 patients aged 64 and older across the United States and European Union, with top-line data expected by the end of the second quarter of 2027.
Each milestone brings our mission closer to the patients living with the disease today.
Retired football professional players carry twice the risk of knee osteoarthritis compared to the general population, and are nearly three times more likely to need a total knee replacement, according to research summarized by OAFI Foundation International.
The disease has reached names the whole world knows. Diego Maradona was diagnosed with severe osteoarthritis in both knees, with his orthopedic surgeon reporting he had no cartilage left and needed prostheses.
Behind him are the 1.1 million professional footballers active worldwide today, playing under the same conditions that raised earlier generations' risk of the disease.
Our Allocetra program was built for them, and for the millions beyond them living with the disease.

People are living longer than ever before, but the quality of those added years has not kept pace.
Between 1990 and 2023, global life expectancy climbed by nearly a decade to 73.8 years. Healthy life expectancy grew by only seven, to 63.1. That leaves 10.7 years, on average, spent in poor health, according to the latest Global Burden of Disease study.
Musculoskeletal disease is the largest single category behind those years of poor health. Nothing else worldwide takes more time from human healthspan.
Osteoarthritis is one of its leading contributors.
Our Allocetra program is being developed to address it at the mechanism level, targeting the inflammation and immune dysfunction that drive joint deterioration, aiming to reach patients before the disease has done irreversible damage.
1 in 5 adults in the US has a form of arthritis.
That is 53.2 million people, according to the CDC. Osteoarthritis alone accounts for 32.5 million of them, more than any other form of the disease combined.
Behind the numbers is a specific population. Arthritis prevalence rises sharply after age 45, and as the older US population grows, the number affected is projected to climb toward 78 million by 2040.
Osteoarthritis is one of the leading causes of mobility loss in the country, and one of the top drivers of joint replacement surgery.
Despite the scale, the treatment landscape has barely changed in decades. Available options treat the pain, modify activity, and end at joint replacement for advanced disease.
Our Allocetra program targets the immune dysfunction behind OA, to reach patients while the joint can still be preserved.

A study published in Nature Medicine in June, led by Washington University School of Medicine in St. Louis, found that people born in the 1990s are biologically older than people born in the 1960s were at the same age.
The gap between how long people live and how long they stay healthy is not a problem that arrives at the end of life. It is arriving earlier, in generations that are not old yet.
That changes what longevity medicine is for. The work is not just about adding years. It is addressing the decline that is already showing up ahead of schedule.
Our quality longevity mission is globally accessible by design.
The clinical program is being developed for the hundreds of millions of people living with osteoarthritis worldwide, because the disease does not stop at any border.
The economic architecture follows the same principle. Enlivex was listed on the Nasdaq, and its equity was tokenized on @Ondo Global Markets, extending exposure to investors outside the traditional US brokerage system.
Both are designed the same way: to reach people, wherever they are.
For a century, osteoarthritis was understood as a wear-and-tear condition.
The joint was thought to age like a mechanical part, breaking down with repeated use until nothing could be done.
Today, inflammation and immune dysfunction are recognized as core drivers of the disease, with joint deterioration shaped by biological processes that were never on the wear-and-tear map.
This changes what is possible. These drivers can be targeted using a therapy, while mechanical wear can only be managed or the joint replaced.
Still, for more than 600 million people living with the disease worldwide, there is no approved therapy that alters its course.
That is what our quality longevity mission was built to change.
Osteoarthritis does not affect men and women equally.
Women account for 60% of osteoarthritis cases worldwide, and the gap widens with each decade from age 40 onward. Knee OA hits women hardest, with prevalence, symptom severity, and progression rates all measurably higher.
For years, this pattern went unexplained under a mechanical view of the disease.
That framing has changed. OA is now recognized as a disorder driven by chronic low-grade inflammation and immune dysfunction, with hormonal and metabolic factors amplifying the risk in women in particular.
Our quality longevity mission is designed to address the disease at the inflammatory and immune level that drives it.

Osteoarthritis is often framed as a condition that takes hold later in life, but its impact can begin decades earlier.
Global OA prevalence in working-age adults has more than doubled since 1990, from 147 million cases to 329 million in 2021. By 2040, the projection reaches 362 million.
This is the population that carries the productive weight of every economy. When OA impacts it, the effect extends to labor force participation, workplace absenteeism, and long-term disability claims.
Our Allocetra clinical program was built for that reality. Our mission is to reach patients before the disease has done irreversible damage to the joint.
