This week, we gathered some of Philly’s most critical voices in emergency medicine for an evening that was equal parts thought-provoking, candid, and inspiring. Against the backdrop of a beautiful space—with incredible wine supplied by our favorite South Jersey vineyards—we tackled the future of EM: AI, workforce challenges, private equity, and the evolving role of the emergency physician.
What stood out most wasn’t just the weight of the discussion but the energy in the room—the passion, the sharp insights, the genuine camaraderie. It was a rare kind of night, one that reminded us all why this specialty matters and why the people in it are so exceptional.
Grateful to everyone who showed up, shared their wisdom, and made it an evening to remember!

Key patterns to rule out in cardiac syncope:
🔹 Brugada 1: Coved STE >2mm (V1-V3) + inverted T wave.
🔹 Electrolytes:
•⬆️K: Peaked T, wide QRS, no P.
•⬇️Ca: Long QT, Osborn J wave.
•⬇️Mg: QT prolongation, arrhythmias.
🔹 WPW: Delta wave, short PR.
🔹 HOCM: Dagger Q waves, LVH, giant T inversion (precordial).
🔹 ARVD: Epsilon wave, prolonged S in V1-V3.
🔹 Trifascicular block: RBBB, LAFB/LPFB, 1° heart block.
🔹 Long QT: >480ms.
🔹 Short QT: <360ms.
🔹 PE: RBBB, S1Q3T3, RV strain.

Brief reminder that the 3 different lumens of a 'triple lumen' central line are *not* equivalent in terms of flow rate.
The distal (brown) lumen usually has a greater diameter and a much higher maximum flow rate than either of the other 2 lumens.
