Lessons for the revolution
Medicine doesn't have a vital sign for aging yet, and proving one is possible had to come first. Cholesterol and A1C are useful because they measure something specific. Nothing does that for aging, which is why Barzilai's centenarian research and the TAME trial were really arguments that aging is a single measurable condition before anyone could build a test for it.
One well-aimed drug can move several hallmarks of aging at once. Targeting a single hallmark, like autophagy or mTOR signaling, changes the others too, which is why a handful of existing drugs already show benefits across diseases that look medically unrelated.
Predicting biological age and proving a treatment changed it are different jobs. Methylation and proteomic clocks estimate how old you biologically are. FAST is built to find the much smaller set of biomarkers that visibly shift within months of an intervention, which is what a clinical trial, or a clinic, actually needs.
None of this requires waiting, and the FDA may not be the obstacle it once was. Several drugs already prescribed for other conditions appear to act on the biology of aging directly, and behavior — exercise, sleep, diet, social connection — remains the surest lever available while the rest catches up.

What if, instead of waiting for diseases to develop, we could target the underlying biology of aging itself?
In the latest episode, Prof. Nir Barzilai discusses the foundation of the field: through geroscience research, we may be able to delay aging, prevent multiple age-related diseases, and extend healthspan and lifespan.
