💬 Editorial by JAMA+ Trials Editor in Chief Alison Huang, MD, MAS, Mara Schonberg, MD, MPH, and JAMA Internal Medicine Deputy Editor Deborah Grady, MD, MPH: Deescalation, discontinuation, and deimplementation trials increasingly evaluate whether reducing or halting interventions—such as medications, screenings, or therapies—provides comparable or improved outcomes for patients, especially when current practices are suspected of limited value or potential harm.
Recent trials in #MultipleSclerosis, #Obesity, #Psychosis, #HeadAndNeckCancer, insomnia, and chronic pain demonstrate that optimal approaches depend on the intervention being withdrawn, patient characteristics, and disease evolution.
Findings indicate that deescalation strategies require careful patient selection, robust evidence, monitoring for recurrence, and shared decision-making to address trade-offs between intervention burden and disease control.
