Its not about more or less fluids. Its about giving patients the right hemodynamic support based on their physiology! Some patients in early septic shock are: 1. Fluid responsive with low SV --> fluids 2. Fluid unresponsive with low SV --> pressors, inotropes 3. Fluid responsive with high SV --> pressors 4. Fluid unresponsive with high SV --> pressors 5. Congested --> inotropes + diuresis 6. Impaired microcirculation with hemodynamic coherence --> fix macrohemodynamics 7. Impaired microcirculation with hemodynamic incoherence --> MAP target, avoid edema, other? (anticoagulation) We need to stop treating "septic shock" as hemodynamic paradigm.
Among adults with septic shock, the use of restricted fluids and early vasopressors did not result in more days alive and out of the hospital through day 90 than the use of liberal fluids and later vasopressors. Full ARISE FLUIDS trial results and Research Summary: nej.md/3PVdwIp

How do you CRT? Microscope slide vs. manual pressure? 🧐 No direct comparison in the literature 🧐 ANDROMEDA-SHOCK used slides, but the use of manual pressure has been investigated more rigorously and in greater detail (studies by Ait-Oufella et al) 🧐 Either is probably fine; manual pressure seems less clunky to me (until proven otherwise) How to measure elapsed time? ⏱️ Most literature & data is based on using a stopwatch ⏱️ A wristwatch with a chronograph function may be the easiest to use How many measurements? 🔁 Precision increases by repeating CRT 2-3 times (vs a single measurement) 🔁 But this is time-consuming and tedious I'd be super interested to hear exactly how people do this. We talk about CRT a lot, but my guess is that everyone is doing it a bit differently.

Some papers are about data. Others reflect a deeply held belief. Pleased to see my editorial “Echoes Without Borders” published in EHJ–Imaging Methods and Practice. 🌍🫀 Talent is universal. Access to education is not. Perhaps our impact as educators should be measured not by how many people we teach, but by how many educators we help create. Knowledge should travel more freely than borders allow. My two preferred papers of life are “the holistic view..(COCC 2012) and the recent “25 years of septic shock resuscitation…(CCF 2026). Science, soul, heart. The first was after 15 years of research in perfusion that started with gut tonometry. The last one “what have we learnt!”

🔹️3️⃣Tolerancia a LÍQUIDOS. 💠 Muchos protocolos de reanimación POCUS..👇🏽 @AndromedaShock @ThinkingCC – at Benito Juárez, Distrito Federal

In 50% of cardiac arrests, CPR is done too high over the LVOT or Aorta which is associated with lower rates of ROSC & lower ETCO2. In this TEE done intrarrest, the CPR is not compressing the LV, and instead is compressing the right of the screen which is aorta. No LV compression = no aortic valve opening = no forward flow = no ROSC.

Nice review article by @Aboumarie (kudos!) examining the evolution, physiological principles, and clinical applications of point-of-care ultrasound (POCUS) and Critical Care Echocardiography (CCE) Some take-home points: * POCUS has expanded from simple bedside "rule-in/rule-out" exam into a comprehensive diagnostic approach ** The "POCUS Pyramid": Effective hemodynamic evaluation relies on integrating three key pillars rather than evaluating an organ in isolation: echocardiogram + lung ultrasound + systemic venous assessment (VEXUS) Key applications in Critical Care: 1. Differentiating shock phenotypes 2. Assessing hemodynamics & preload responsiveness 3. Evaluating hypoxemia You may find useful the proposed algorithms for the evaluation of patients with hypotension and/or hypoxemia:

Not everyone performs #VExUS yet since it's a relatively advanced #POCUS application. But we can often get an early clue from routine radiology studies we already order, such as liver or kidney Doppler. @ChailShah does a great job highlighting this perspective in a narrative review written in collaboration with his radiologist father (& me). 🔗wjgnet.com/1949-8470/full/v1…
