👉Just published in @NEJM: STAREE Trial
☝️At last, robust randomized evidence on statins for primary prevention in older adults.
🔹 9,971 adults ≥70 years, without cardiovascular disease, diabetes, or dementia
🔹 Atorvastatin 40 mg vs placebo
🔹 Median follow-up: 5.9 years
🔹 Baseline LDL-C: ~127 mg/dL NEJMoa2607314.pdf
👉Key findings:
🔹 30% reduction in major cardiovascular events
HR 0.70 (95% CI 0.61–0.82; P<0.001)
🔹 NNT = 37 over 5.9 years
🔹 MI: HR 0.57
🔹 Coronary revascularization: HR 0.57
🔹 Benefit was also observed in participants ≥75 years
👉Importantly:
🔹 No increase in dementia (HR 1.03)
🔹 No significant effect on all-cause mortality
🔹 No improvement in overall disability-free survival
🔹 Serious adverse events were similar between groups.
☝️Bottom line:
Age alone should not be a reason to withhold LDL-C lowering. Even in healthy adults ≥70 years, lowering LDL-C translates into substantial cardiovascular benefit.

Help Shape Best Practices in Athlete Cardiovascular Care
Cardiologists and physicians who treat athletes or highly active individuals are invited to participate in a brief research survey from the Cardiovascular Performance Program at Massachusetts General Hospital. The survey explores how providers approach cardiovascular prevention in this patient population — and takes only 5–10 minutes to complete.
Your input will help improve collective understanding of care practices in this specialized area. Participation is voluntary and anonymous.

A new scientific statement from the @American_Heart affirms the ongoing importance of physical activity in #Obesity care, even as #GLP-1 drug therapies become widely used.
In the latest JAMA Medical News podcast, Jennifer Abbasi and Samantha Anderer highlight evidence that physical activity reduces blood pressure, improves cholesterol, enhances insulin sensitivity, and maintains muscle quality—health benefits that extend beyond weight loss.

Despite high levels of fitness, tactical athletes—including military personnel, firefighters, and law enforcement officers—face a substantial burden of atherosclerotic cardiovascular disease (ASCVD) risk that often goes unrecognized.
This state-of-the-art review highlights how traditional risk factors such as hypertension, dyslipidemia, obesity, insulin resistance, and sleep disorders intersect with unique occupational exposures—including shift work, heat stress, smoke inhalation, and performance-enhancing substances—to increase cardiovascular risk. The authors argue for a shift from symptom-driven evaluation to proactive screening, aggressive risk factor management, and selective use of coronary artery imaging.
Key takeaways:
🔹 Sudden cardiac death remains the leading medical cause of duty-related mortality across tactical professions
🔹 High fitness levels may mask significant cardiometabolic risk
🔹 Coronary artery calcium (CAC) testing may help identify subclinical disease in selected individuals
🔹 Prevention strategies should be tailored to the unique demands of tactical occupations and operational readiness requirements
