📌 Randomized Trial of Hypofractionated Postmastectomy Radiation Therapy in Women With Breast Reconstruction (RT CHARM, Alliance A221505)
🔗 ascopubs.org/doi/10.1200/JCO…
🔬 This randomized phase III trial enrolled 898 women undergoing postmastectomy radiotherapy (PMRT) with planned immediate or delayed breast reconstruction.
Patients were randomized to:
🔹 Hypofractionated PMRT: 42.56 Gy/16 fractions
🔹 Conventional PMRT: 50 Gy/25 fractions
📊 Key findings (825 evaluable patients):
👉🏻 Reconstruction complications at 24 months: 12.3% vs 10.2%, respectively (noninferiority p=0.0001).
👉🏻Estimated LRR at 3 years: 1.5% vs 2.2%, with no statistically significant difference.
👉🏻 Acute radiation dermatitis: significantly lower with hypofractionation (G2: 20% vs 42%, p<0.0001; G3: 1% vs 3%, p=0.048).
👉🏻 Reconstruction timing: delayed reconstruction was associated with higher complication rates compared with immediate reconstruction (OR 2.96; p=0.004), regardless of fractionation.

The 2nd Florence International Consensus on combining radiation therapy with new systemic therapies has wrapped up in beautiful Florence, Italy 🇮🇹 #FlorenceConsensus26
This year the focus went beyond breast cancer to include:
🔹 Head & neck cancer
🔹 Kidney cancer
🔹 Prostate cancer
It caps a year and a half of work by a multidisciplinary team tackling one of the most practical questions in oncology: how do we integrate RT and systemic therapy the right way?
💡 When should we pause treatment?
💡 When should we combine?
💡 How do we reduce adverse events and boost effectiveness?
The statements and recommendations, endorsed by @ESTRO_RT, aim to guide professionals and help us inform our patients properly.
Patient advocacy was a core part of the consensus, because clear communication with patients is key to optimal treatment. 🤝
Radiation oncologists, medical oncologists, surgeons, nurses, advocates: this conversation needs all of us. Be part of the next edition and help shape how we treat our patients. 👇

Should we ablate oligometastatic breast cancer? NRG-BR002 in JCO (Chmura et al.; @Docace911 @IBCradiation) gives us randomized data. 🧵 #bcsm
125 patients with ≤4 metastases and no progression on up to 12 months of first-line systemic therapy were randomized to SBRT or surgery to all sites (93% got SBRT) plus systemic therapy, vs systemic therapy alone. 79% had HR+/HER2− disease.
🚫 benefit. Median PFS was 19.5 vs 23.0 months (HR 0.92, 95% CI 0.57–1.47), and OS HR was 1.07. The trial didn't advance to phase 3. No grade 5 toxicity 👍🏻
⚡️⚡️The interesting part: an exploratory analysis suggests patients with low or absent CTCs at baseline may get a PFS benefit from ablation. That makes biological sense, since these may be the patients with truly limited disease and less micrometastatic burden.
Bottom line: no routine consolidative SBRT for oligometastatic breast cancer outside a trial. CTC-selected trials could be the next step.

Turns out IR ablations are toxic. Probably less effective. I’m not surprised but would be kicked out of tumor board for making such claims.
