Thank you @anaselgenidy1
The session was great with several discussions around immune reset, naive vs memory B cell roles post immunotherapy and CAR-T therapy.
Excellent presentation by Dr. Irani @Mayo_AING at #ANA2026 on a fundamental question in neuroimmunology: What causes autoimmunity in humans?
Fascinating insights into how central and peripheral immune tolerance shape CASPR2-directed autoimmunity.

Excellent review article on MOGAD MRI Imaging.
Congratulations to all the authors.
Delighted to share our international review on MRI Characteristics of #MOGAD! ➡️ Key imaging features are highlighted across the brain, optic nerves & spinal cord including lesion dynamics ➡️ Illustrative figures help show its full imaging spectrum👇

This retrospective cohort study with a preplanned analysis found that prednisolone ≥10 mg treatment for ≥3 months after the index event was associated with a lower likelihood of relapsing MOGAD: hubs.la/Q04w51LN0
🧠 The “Kiss Swan” sign is a symmetric medullary T2 hyperintensity seen in adult leukodystrophies, particularly Alexander disease (GFAP). It may guide genetic testing but is not pathognomonic. 👌

Autoimmune Parkinsonism: When Should We Suspect It?
▶️ Antibody-associated disease: only 1.3%.
▶️antibody screening: not recommended.
▶️Red flags: rapid course, inflammatory CSF, atypical MRI, or cancer.
▶️Anti-IgLON5: think sleep apnea, bulbar/neuromuscular signs, or chorea.

This study looked at the incidence of dementia, stroke, epilepsy, and Parkinson disease in older Veterans before and after acute #TBI to determine whether these conditions are bidirectionally associated with TBI: hubs.la/Q04pxH5f0
▶️SCLC with anti-GABABR, anti-Hu, and no PNS showed distinct molecular profiles.
▶️Anti-GABABR tumors were associated with 5q gain and KCTD16 overexpression.
▶️Anti-GABABR showed B-cell signatures, while anti-Hu showed T-cell/IFN-γ signatures.

Septin autoimmunity has a true queen! 🟢🟢
Congratulations Friederike Arlt at @MayoClinicNeuro and @ChariteBerlin on this outstanding study in @Brain1878 identifying antibodies against the septin complex in lower motor neuron disease, suggesting a potentially antibody-mediated incisuropathy due to septin binding🔝
And you’ve just presented new phenotypes in the expanding field of septin antibodies, including brainstem syndromes, encephalopathies, and myeloradiculopathies, likely reflecting their broad expression throughout the nervous system.
Can’t wait to see what’s next‼️

ICI-neurotoxicity: long-term outcomes
• Recovery beyond 3 months is rare
• Relapses: 15% mostly year1
• 40% have 1-year sequelae
• Immunosuppression>1 year may be unnecessary
• Only 5% relapsed after rechallenge
Rechallenge appears safe after recovery

➡️Asymmetric weakness, paresthesias, severe neuropathic pain, sensory ataxia, bladder dysfunction, orthostatic intolerance
➡️MRI spine: longitudinally extensive, tract-specific T2 lesions

▶️about two-thirds achieved substantial or full recovery over long-term follow-up.
▶️Older age and higher NEOS scores increase mortality

This constitutes a significant diagnostic challenge, particularly in neuromuscular disorders, where many paraneoplastic phenotypes are non-classical and often overlap with other neurological conditions.
