The Food and Drug Administration (FDA) has approved the immunotherapy drug atezolizumab in combination with chemotherapy for some patients with stage 3 colon cancer following surgery. The treatment is for patients whose tumors have deficient DNA mismatch repair, or dMMR. The approval is based on results from the ATOMIC clinical trial led by Mayo Clinic. Earlier this week I said the most important development in IBD of the last decade was... biosimilars. Here's the other half of the story. 20 new molecules in Phase 2 or later. Eight targets, plus dual-target combinations. If their trials succeed, up to six could be approved in Europe by 2031. TL1A is entering phase 3 readout time. Oral IL-23R blockade is in Phase 3. Co-formulated combination therapy is in Phase 3. Look at phase 2 and the pipeline is stacked. There’s even an anti-fibrotic for stricturing Crohn's. Biosimilars bought the headroom. This is what fills it. And it is very exciting. The limiting step now isn't the science. It's trial delivery: opening sites, finding eligible patients, research teams fitting studies around full clinical jobs. The good news? That's the part we can fix. Faster set-up, and every eligible patient offered a trial. That's how 2033 becomes 2031. Honestly, this is extraordinary.

Very timely discussion on @mondaynightibd. Exciting new monoclonal antibodies are getting closer to entering the IBD treatment armamentarium: anti TL1A therapies. Several molecules are now competing in this race, with subtle but potentially important molecular differences in how they interact with TL1A and the TL1A DR3 pathway. So far, I would not call the efficacy results earthshattering. The bigger questions may be what makes this pathway truly different: 🔬 Can TL1A inhibition make a clinically meaningful dent in intestinal fibrosis, rather than simply control inflammation? 🧬 Could its future lie in combination therapy, as Spyre is exploring by combining TL1A blockade with other pathways? And one question I find particularly intriguing: 💭 Could TL1A inhibition eventually have a role in collagenous colitis, given the biology of mucosal inflammation and collagen deposition? The TL1A story may ultimately be less about another effective biologic and more about whether it can do something our current therapies cannot. This thread is a great summary of what we know so far for the anti-TL1A antibodies for the treatment of #IBD Once we see the phase 3 data on these molecules we’ll have a better sense of how they fit in the algorithm

In February 2024, Propel a Cure awarded a 2-year, $100,000 Trailblazer Grant to Promakhos Therapeutics to advance an innovative approach to treating #Crohns disease. Promakhos is developing an oral, non-immunosuppressive precision therapy designed to restore a natural biological signal that appears to be missing in Crohn’s. With support from Propel a Cure and other funding sources, the team has made significant progress. In preclinical studies, the therapy has shown promising results by promoting intestinal healing, reducing bowel inflammation, improving the gut microbiome, and helping clear invasive microbes. The candidate drug also has a distinct mechanism of action that works upstream of current therapies and is paired with a biomarker strategy to help identify patients most likely to benefit. In Spring 2026, Promakhos co-founder and CEO @Katerina_Hatzi received the prestigious Harvard President’s Innovation Challenge Award, recognizing groundbreaking solutions to global challenges. The next milestone is advancing this promising therapy into human safety studies, bringing it one step closer to patients living with Crohn’s disease and others affected by inflammatory diseases.

At @WoolstonLabNEU, a recipient of a Propel a Cure grant in partnership with the Mendez Family Foundation, animal experiments examining the long-term effects of modulated #sulfide on gastrointestinal physiology are continuing. Researchers hope these studies will reveal to what extent raising sulfide levels in otherwise healthy mice contributes to the onset of inflammation, providing new insights into the role of microbial hydrogen sulfide in #IBD. The team has also reached an exciting publication milestone by submitting its first manuscript about the engineered bacterial strains used in the project to a scientific journal, and has received favorable comments from peer reviewers. They are now working on incorporating the feedback into a revised paper and are hopeful the published paper will be available online soon. Northeastern University recently produced this 60-second video clip explaining this exciting project. To view, please click here: m.youtube.com/shorts/HRvwIQv…

A Milestone Year for IBD Research 🎉 Our two scientific journals just hit new highs in their "impact factor" — a measure of how widely research is cited and used by the scientific community, signaling its influence on the field. 🔹 Inflammatory Bowel Diseases®️: 5.5 (up 28% from last year) 🔹 Crohn's & Colitis 360: 3.0 (up 15% from last year) The takeaway: research published in our journals is reaching more doctors and scientists than ever — accelerating progress toward better treatments and cures for Crohn's disease and ulcerative colitis. Thank you to our editors, reviewers, and researchers for driving this work forward.

There is nothing quite like the joy of seeing healing. Behind every healed intestine is a patient who may finally be sleeping through the night, returning to work, enjoying meals, and living life without constant fear of a flare. This is why we strive for more than symptom relief. We strive for deep healing. Every healed mucosa represents renewed hope. IBD Library™
