🚨 Temporary MCS in Cardiogenic Shock: What’s New in the 2026 ESC Consensus?
🔹 Phenotype before device: SCAI + SHARC + hemodynamics
🔹 Impella: Survival benefit in selected STEMI-CS (DanGer Shock), but more complications
🔹 VA-ECMO: No benefit with routine use in AMI-CS (ECLS-SHOCK)
🔹 Biventricular support: Promising physiology, limited randomized evidence
🔹 Define the exit strategy BEFORE initiating support
💡 More flow ≠ better survival.
The future of MCS lies in selecting the right patient, device, and timing—not simply escalating support.

In balloon-expandable TAVI, prolonged balloon inflation was associated with favourable valve expansion, haemodynamic performance, and early clinical outcomes, suggesting a potential alternative to routine double-tap in selected patients.

In the prespecified HBR substudy of SELUTION DeNovo, a sirolimus-eluting balloon strategy with provisional stenting was compared with systematic DES implantation.
At 1 year, rates of target vessel failure, major bleeding, and net adverse clinical events were numerically lower with the SEB strategy.

The question:
Did 1 Stent PCI have lower 2-year MACE than 2 Stent PCI cause the side branch was less important?
🫀True #bifurcation #PCI remains challenging. 🔬 In a #JACCINT study, 1-stent PCI had lower 2-year MACE than 2-stent PCI (9.0% vs 13.9%). 👁️#OCT-guided DK-crush had fewer events than angiography-guided PCI. 📊 Complexity matters! 🩺

When Is a Coronary Artery “Small”?
RCTs suggest DCBs are non-inferior to DES in small vessels. But where’s the cutoff?
The magic number for long-term stent patency is an MSA of 5.5 mm², corresponding to a vessel diameter of 2.6 mm. See the sizing chart below.
The question of the day: Is a ref. vessel dia. (RVD) of 2.75 mm the tipping point for choosing DES over DCB? 🤔

If 3mm DCB fully yielded without recoil in this young man without heavy calcium then that’s an MLA of 7mm2. It’s adequate.

Cardiac abnormalities in a young athlete with severe pectus excavatum: a case report of a diagnostic loop between anatomical distortion and cardiomyopathy
Can severe pectus excavatum mimic—or mask—underlying cardiomyopathy? This case highlights the diagnostic challenges of evaluating cardiac abnormalities in young athletes, emphasizing the importance of multimodality imaging and careful differential diagnosis to distinguish anatomical distortion from true myocardial disease.
