Four overlooked risks stand out. Long-term acid suppression can impair magnesium absorption, sometimes dropping levels low enough to trigger muscle spasms, seizures or dangerous heart rhythms. PPIs can also provoke silent acute interstitial nephritis: creatinine rises while the patient feels well, fever and rash are often absent, and delayed recognition may leave lasting kidney damage. Stomach acid is required to release vitamin B12 from food; years of suppression can produce deficiency that appears as anaemia, tingling, poor balance or unexplained memory problems. Finally, reduced acid weakens a natural barrier against swallowed microbes, modestly raising the chance of gut infections including C. difficile, especially during antibiotic use or hospital stays. He stresses that PPIs remain excellent at preventing ulcers and life-threatening bleeding and that some patients genuinely need them indefinitely. The real problem is automatic refills. Sudden cessation often causes rebound acidity, so any change should be discussed with a clinician who can confirm whether the original indication still exists and whether the lowest effective dose is being used. These points align with current evidence. Associations with hypomagnesemia, B12 deficiency after two or more years, enteric infections and kidney injury are repeatedly documented, though most data remain observational and subject to confounding. A 2026 Nordic study found no excess gastric-cancer risk after rigorous adjustment, offering reassurance on that particular fear. Newer potassium-competitive acid blockers may carry a more favourable renal profile in patients who already have chronic kidney disease. Guidelines continue to recommend annual review, dose reduction or a structured taper rather than indefinite automatic continuation. In my view this timely and balanced. PPIs save lives when used for the right reasons and the right duration; the greater public-health issue is unexamined long-term prescribing. Patients should not stop the tablets themselves, but they should ask, at least once a year, whether they still need them.
