🫁 HARMONi-2 now has an overall-survival result, and it strengthens the case for dual PD-1/VEGF targeting in first-line NSCLC. At WCLC 2026, ivonescimab significantly improved survival compared with pembrolizumab monotherapy in previously untreated PD-L1–positive advanced NSCLC: OS: 30.8 vs 22.6 months, HR 0.73 24-month OS: 57.9% vs 48.0% 36-month OS: 45.0% vs 33.1% This follows the previously reported PFS advantage: 11.1 vs 5.8 months, HR 0.51. The survival signal was particularly notable in patients with PD-L1 TPS ≥50%, where the OS HR was 0.58, although this subgroup analysis was descriptive rather than independently powered. Ivonescimab was associated with more grade ≥3 treatment-related toxicity than pembrolizumab, particularly VEGF-related proteinuria and hypertension, highlighting the need to evaluate efficacy and safety together. The broader significance may extend beyond one drug: HARMONi-2 provides phase III evidence that combining immune checkpoint inhibition and VEGF blockade within a bispecific antibody can outperform PD-1 monotherapy.
