In CHIP-BCIS3, elective MCS use was associated with higher CV ☠️ in patients undergoing HR-PCI, when compared against standard care. In our latest analysis, we investigate whether procedure-related myocardial injury was the missing 🧩 linking MCS use with CV ☠️ .

Contemporary practice in HR-PCI (as per PROTECT III etc) is to remove the Impella (CP) after PCI in the cath lab - this is the strategy evaluated by @CHIP_BCIS3. Would post-PCI support help? 🤷🏽‍♂️ … but higher vascular comps/bleeding will need to be offset by possible gain

Also, based on clinical features, the trial eligibility criteria identified a #SCAI A (at RISK of cardiogenic shock) cohort… v low cardiac output, limited reserve, frequent procedural hypotension. ➡️ @CHIP_BCIS3 criteria can be used to define High-Risk PCI

We included ppMI in the composite primary outcome as our hypothesis was that mAFP use would protect the myocardium from injury. But it turned out myocardial injury was MORE likely with mAFP. ppMI was only used to determine wins in pairs tied on MACE… and favoured standard care

The pre-specified hierarchy of components in the primary outcome ensured mortality “trumped” other MACE, which trumped ppMI. Mortality was higher with mAFP and had the greatest impact on the Win Ratio (40% of pair-wise comparisons decided by death … and favoured standard care)

P Vranckx, J Møller, G Tavazzi say routine use of elective MCS cannot be supported on the basis of @CHIP_BCIS3 but note modest sample size. doi.org/10.1093/ehjacc/zuag0… But isn’t it events that determine power? 221 of 300 pts experienced a composite primary outcome! (129 excl. ppMI)
