We (or YOU, really!) have thoughts. Some highlights in diagnostic stewardship (removing UA/cx from order sets), ER interventions on following up on UA cx, and a "mindful pause" on diagnosis.... #ASPchat

Plenty of favorable data of linezolid in endocarditis. Be careful of iv therapy in endocarditis. Staph failure rates with iv therapy are like 30%! 😉

Really enjoyed this concise personal perspective on the "oral vs IV" antibiotic debate, in particular the author's experience with home OPAT.
(It wasn't good.)

No fan of OPAT (an understatement!), I read with great interest this study on completing endocarditis treatment with oral Rx assembled by @SarahRawi1 @bradspellberg et al in @CIDJournal.

The legendary Burke Cuhna published a review of doxy that indicated good kidney/urinary levels if GFR good. Sounds like levels fall substantively with poor GFR. pubmed.ncbi.nlm.nih.gov/7048…

These are GREAT! A few more mini-myths:
- if PCN allergy➡️allergy to ALL beta lactams
- if SSRI➡️never linezolid
- FQs: too toxic to use in outpt setting -- ever
- Never use nitrofurantoin w/⬇️GFR
- Linezolid is 💰
- Doxy is a "weak" abx
- Dental work + prosthetic joint = Abx

#1/2 There is this fascinating, widespread cognitive dissonance around the safety of oral therapy. Uncertainties about exposure and safety of oral therapy? What about exposure and safety of IV therapy? Ain't no data for the latter. So why is oral held to a different standard?
