Useful context for discussions about chemogenetics trials in China and U.S.

As a medical technology, CGT (cell and gene therapy) can conduct IITs (investigator-initiated trials) even before the product is approved for market — it only needs to be filed with the hospital's supervising authority, registered on ChiCTR, and approved by the hospital's ethics committee. This creates a regulatory gap. The "Order 818" policy, which took effect May 1 this year, has tightened oversight of this type trials to some extent. Of course, CGTs also can be as drug to file IND applications to NMPA, and conduct registratory study and approved as drugs. For non-medical-technology drugs, however, IITs still require NMPA market approval first — for example, real-world studies exploring expanded indications. Separately, Hainan's Boao Lecheng pilot zone serves as a testing ground, allowing products already approved overseas but not yet approved by the NMPA to be used in designated pilot hospitals after approvals within the zone, enabling real-world studies to be conducted there.

Worth noting that over $100M was invested into chemogenetics in the US based on two companies: Redpin and CODA. CODA tried to replicate Redpin’s technology but ran out of time and money. Redpin was acquired by Kriya and the Redpin tech (developed by my lab) is in clinical trial.
For years I’ve been asking my neuro colleagues why we hadn’t done clinical chemogenetics yet. Most said too pricey small or risky. Yet clearly the tech and the need are there. My take: VCs in the US are too big now — only social media (prev) or AI (now) have the scale they want.

There was a Bay Area co called Coda doing this a few years ago. Not sure if they made it into humans

The risk adjusted economic issues partly come from uncertainty about regulatory path. Small molecule + gene therapy was not an established path with regulators. Our PSAM/varenicline chemogenetics had green light in 2023 but trial finally started this year clinicaltrials.gov/study/NCT…

Some pics from @cpnoct, @adhikari, and @BrainCaRMA's visit to Seoul. Learned so much at both SNU Symposium on Neural Circuits for Thermoregulation (and feeding behavior ;) + #icomes2025. Thanks for inspiring science, great conversations, and fun times!

1/ 🧬 The Cocaine Cure What if the same drug that hijacks your brain could be used to treat its own addiction? Scientists are engineering ion channels that only turn on in the presence of cocaine, flipping the reward circuit on its head. Here’s how it works, and why it matters 👇🏼 🤠Be sure to read to the end if you are also into Fentanyl and Meth

We used protein engineering to develop cocaine-gated ion channels. Our approach simplifies chemogenetic applications for cocaine addiction by having cocaine itself activate inhibitory and/or excitatory channels. Collaboration between @mikelides @BrainCaRMA rdcu.be/eCJK4
