INTRAGO-II is now published in @TheLancetOncol and the result is what decades of evidence predicted: dose escalation does NOT improve survival in glioblastoma.
Here's what the data show:
🔴 90 Gy 3D-CRT? No OS benefit (Chan, JCO 2002)
🔴 SRS boost (15-24 Gy)? No OS benefit (RTOG 93-05, Souhami, IJROBP 2004)
🔴 Brachytherapy to 110 Gy? No OS benefit (Welsh, IJROBP 2007)
🔴 MRS-guided 72 Gy + TMZ? No OS benefit (SPECTRO GLIO, Laprie, Neuro-Oncology 2024)
🔴 IORT 30 Gy boost? No OS benefit (INTRAGO-II, Giordano, Lancet Oncol 2026)
🟡 Protons 75 Gy? Statistically met a relaxed threshold (α=0.15) but clinically negligible ~2.5 wk gain (NRG-BN001, Mehta, ASTRO 2025)
GBM cells infiltrate centimeters beyond what we can see or target. No amount of local dose intensification overcomes that biology. As INTRAGO-II's own authors conclude: this "questions the value of further local dose intensification in resectable glioblastoma."
We owe our patients innovation, not repetition. It's time to stop repackaging the same failed hypothesis with a different modality and redirect resources toward approaches that address GBM's fundamental biology

The PERSIAN randomized phase II trial evaluated apalutamide ± SBRT in oligometastatic hormone-sensitive prostate cancer. Biochemical response rates were high in both arms, with no significant differences overall. A consistent numerical trend favoring SBRT was observed, particularly in patients with higher disease burden. Treatment was well tolerated, with limited interruptions. These findings suggest a potential role for SBRT alongside systemic therapy in selected patients.
