Prescribing vaginal estrogen to women with rUTI halves the risk of hospital admission and reduces sepsis incidence. Consideration of estrogen therapy could be crucial in reducing serious adverse outcomes and improving patient management strategies. is.gd/3NUPnQ

I don’t manage anyone’s hair, I’m not a dermatologist and don’t give out minoxidil or finasteride for alopecia. I give my recommendations when patients ask and I tell them to go talk with their dermatologist about their options. They can make their own decision, but I will always give them my opinion from the data I know and the patients i manage. No one is being disserviced other than the people who are provided these meds without being counseled about the risks of these medications, which most are not.
Regarding the fertility aspect, I have reviewed multiple large data set abstracts on its impact on fertility at the AUA this year, all which will hopefully be published as final manuscripts sooner rather than later. There is more data coming out.

I think that finasteride is one of the worst medications we can give young healthy men. More and more data is coming out suggesting an impact on fertility and sexual health. I think for the right patient to manage BPH it has a role, but a limited one at that.
Personally i recommend my patients avoid finasteride at all costs if possible. For hair loss use topical or oral minoxidil.
Chronic low-dose finasteride (1mg) use, common for hair loss, does not increase BPH or prostate cancer diagnosis risk. Key insight for urologists: prolonged low-dose appears safe for patients without pre-existing prostatic disease. tinyurl.com/2ywqfcr2 @UroOnc

You pay to publish YOUR research.
You pay extra to make YOUR research “open access” so the public can read it.
Then publishers sell YOUR research—and often your peer review labor—to AI/LLM vendors for massive licensing deals.
Academia…what are we doing here?
Researchers generate the ideas, conduct the studies, write the manuscripts, review the papers, and often even fund the work through grants or taxpayer dollars.
Yet the value extraction happens elsewhere.
At some point, academics need to stop treating this system as immutable.
Researchers should be charging the journals. We should stop participating in the racket.

On the Hill this week, we heard how insurers are standing between patients and care.
New data shows 70% of patients are initially denied coverage for a prescribed medicine, delaying or preventing treatment.
Patients should not face red tape. It is time to put them first.

Gene Braunwald died at 96.
In a normal world, this would be front page of the NYT and WSJ.
The father of modern cardiology.
Fled the Holocaust as a boy
Went on to save millions of lives
The most important clinician-scholar of the last 50 years. We should all know his name
The American Heart Association mourns the passing of the legendary cardiologist Eugene Braunwald, M.D., widely recognized as one of the most influential figures in the history of cardiovascular medicine. Over seven decades, his work reshaped the understanding and treatment of heart disease, leading many to call him the father of modern cardiology.
Braunwald was a lifelong contributor to the American Heart Association, helping advance its research and scientific mission, and was honored with some of the Association’s highest honors for his lasting influence on cardiovascular care and research. His influence extended well beyond his own discoveries, as generations of Association‑supported investigators, clinicians and academic leaders were trained by Braunwald or guided by the clinical trial standards and mentorship models he helped establish.

Trimix is very different from cialis and Viagra.
The only time I heard anyone getting it from orals is from gas station pills which has literally 50-100x dosage of Viagra or cialis. Standard dosage i honestly don’t think it’s possible and not knocking your 1 month experience but I’ve never heard of a urologist report it.
PDE5i (Viagra, Cialis) do not cause priapism. In clinical trials over many thousands of men there were zero reported cases. In fact, urologists use them to TREAT recurrent priapism.
Free event tonight at Women's Club of South Pasadena! Please come if you are in the area.

Rebecca Archer’s daughter died of measles that she contracted before she was old enough to be vaccinated. “Parents must realize that refusing vaccinations doesn’t just put your own child at risk. It puts other children at risk,” Archer writes. nyti.ms/3QizMM9

NOT FUN FACT—half of all U.S. war casualties during WWI were due to influenza. Today—Trump’s War Secretary remove the flu vaccine requirement for all U.S. military. Plus, bragging about this to all enemies is just absolute idiocy.
