What is high dose opioid prescribing? arthriticchick.substack.com/… in 2010 200MME was considered a moderate to high dose. Now 60MME is considered high dose. Not because of new evidence, but becuase of politics.

Haven't been here for a long time...but I'm catching up. Good to see an honest headline for once "Pain Reprocessing Therapy may help mild pain" scimex.org/newsfeed/pain-rep… instead of the usual sensationalistic claims that are untrue and harm people who live with secondary pain.

Do you know the difference between chronic primary (nociplastic) pain and chronic secondary (nociceptive or neuropathic) pain? You should. You'd be amazed how many 'pain experts' don't. This is why it matters. open.substack.com/pub/arthri…

Correcting the record – the general public do not believe doctors would force taper opioids. This is how forced tapers lead to pain crisis, suicide, and death. open.substack.com/pub/arthri…

Study on reducing ‘opioid misuse’ in women post caesarean. But how many women abused their opioids? Spoiler: Less than a quarter of one percent So why are we denying people access to opioids? open.substack.com/pub/arthri…

Recent study shows that opioids given in the ED for pain do NOT lead to addiction. Addiction rate was about one quarter of 1%. This is consistent with studies in chronic pain. So why are we denying access to opioids for those who need them ? arthriticchick.substack.com/…

I haven't done much work lately because I've reached breaking point with constant, severe pain. There is no help. I have tried. And no, going to the ED would NOT help. They would treat me as a drug seeker and give panadol. open.substack.com/pub/arthri…

Is it true that all or even most 'chronic pain' is due to an oversensitised nervous system? No, its not. Please stop saying that it is, cos this myth is being used to deny people with severe pathological pain access to safe EFFECTIVE treatment open.substack.com/pub/arthri…

More manipulating the data on opioids...the June 2020 TGA regulatoin changes were NOT a success. And Dr Verhagen is twisting the data in a blatent career building media grab...forgeting or not caring of the harm done to people who live with severe pain! arthriticchick.substack.com/…

IVIG infusion day today. 6 hrs attached to an IV. I hate infusions. I can't move. Stuck in a chair. WORST thing for pain. I need to be able to move. My dose has been increased. Here's hoping the extra bottle is worth it. #CIDP

These photos say it all. On the left, on approproate dose of opioids. Fit, active, social, employed, happy. On the right, after being force tapered: Obese, unable to walk unassisted, living in poverty on disability, no social life This is what forced tapers do to a life.

Before and after. My life BEFORE - fit, active, healthy, social, employed. My life after taking pain meds away...no life Everyone is so focused on pot dangers of opioids. Everyone IGNORES the very real dangers of untreated pain. Please read, share arthriticchick.substack.com/…

This. First ensure there is not a missed differential diagnosis. So many people are suffering because their physio thinks their pain is due to psychosocial causes. Weeks, months or even years later they are diagnosed with ank spond, or a fracture or something pathological.
TOP TIP: If you have a patient who presents with an MSK issue thats been ongoing far beyond the normal expected timeframe… First ensure there is not a missed differential diagnosis, eg rheumatological, neurological, vascular etc! But then look for iatrogenic and socioecominal factors have may have prevented time helping, eg misinformation, negative narratives, lifestyle, self efficacy etc!

OPAL is fatally flawed and totally falsified 'evidence' and drew conclusions the evidence does not support! As @asafklaf says, he just blocks everyone, rather than addressing the critique. If his evidence is so good, he should be happy to defend it. Should be easy!
Prof. Maher he should be ASHAMED of himself for blocking half of this platform for calling his bogus trial and for this puny attempt to spin the onus of proof. Those authors bluntly ignored the rules of logical and scientific inference. Their trial is only “evidence” that one type of opioid not indicated for acute pain is ineffective for acute low back and neck pain. His attempt to shift the onus to the skeptics and critics is pathetic which had no hypothesis.

THIS! THe OPAL study is perfect example of science gone mad! A trial designed with a predetermined result and conclusion..using a slow release opioid that is not indicated, and never used, not representative of clinical practice and more effort spend on marketing than science!
The Lancet is the no.1 medical journal in the world. So, when they published the flawed OPAL trial a year ago we felt we had to respond. The authors of the trial claimed their study showed “opioids are no better than placebo for acute low back and neck pain” and that “guidelines for opioid prescription should be changed due to their findings.” This couldn’t be further from the truth and a massive disservice to patients! Too bad one of the authors of the paper Prof. Chris Maher relentlessly blocked me and many other patient advocates here for merely mentioning the flaws. Special thanks to Dr. @jeubanksMD for his contribution.

I have CIDP, extremely painful neuropathic pain. Opioids, oxycodone, is the only thing that helps. Opioids do work for neuropathic pain. I've been force tapered to a very lose dose, and now only have a few hours a day of relief.
#chronicpain For those of you with painful peripheral neuropathies including small fiber neuropathy what seems to work best. Please retweet
