One of the most important findings relating to MRD at #ASH23: In an older transplant-ineligible patient population, an intensive quaduplet regimen with dara/carf/len/dex had the "highest" MRD negativity rates... But the lowest survival at 18 months! shorturl.at/sFUV0

"Instead of providing free coffee and a piano in a soaring, art-filled marble lobby, how about... making it easier for pts to schedule appts? Shortening wait times? Paying for adequate staffing on nights and weekends? ending 2 day stays in EDs when all inpt beds are full?" theatlantic.com/health/archi…

Oof. 😓 Burnout among oncologists has increased from 34% to 59% over the past 10 years. 22% likely to reduce clinical hours in the next year (up from 16%). #ASCOQLTY23

If you write a protocol for myeloma that requires 12 bone marrow biopsies in 5 years I’m going to assume that you no longer see patients.

I’m waiting for ASH and ASCO to act. Letters and surveys are a start but not the end. We want results. Change. #EndMOC #MedTwitter

A negative Covid antigen test is less reliable these days. In this study, symptomatic, PCR+ people didn’t reliably test positive by antigen test until the 3rd, 4th, or even 5th day of symptoms. Sensitivity Day 1: 30%-60% Day 3: 60%-75% Days 1-7: 60%-80% academic.oup.com/cid/advance…

I am still coming back for medTwitter updates, but I really hope we transition to a healthier platform. Your curated Twitter feed may be fine, but our eyes and engagement are supporting a cesspool that is threatening democracy and scientific progress.

Another @ABIMcert MOC LKA heme block done today. Another 7/30 "test" questions...many ?s about incredibly rare scenarios (2 about same one)...one where only reference was from 1980, many >20 yrs old...2 undermined by recent data. Come on. This must end. @AaronGoodman33 @VincentRK

Well it finally happened. Not to me but to my parents. They both got their Paxlovid prescriptions but their different experiences showed how YMMV depending on your health care provider. First shout out to the good folks at Kaiser Permanente. KP's system worked smoothly and well

Replying to @doctorwes @ABIMcert
For ABIM, I was “not certified, not participating in MOC,” as of 2 AM last night. Paid my extortion fees, and past due items. Now I’m Certified again this morning. $1000 bucks. What a waste

In post-Dobbs America, some girls can travel hours to get an abortion; some can't. This is the story of one of them. She was raped. The nearest abortion was nine hours away. Now, she's a mom. Soon, she'll start seventh grade. time.com/6303701/a-rape-in-m…

I invite @RichardBaron17 of @ABIMcert on #HealthcareUnfiltered 2 answer Qs all MDs have about the value of recertification. @AaronGoodman33 efforts R excellent and I urge all to put similar pressure on their own hospitals 2 not mandate the recertification for staff privileges.

I came to realize that MOC was primarily a cash grab, with no data on efficacy beyond CME. Initial certification, I still support. I’ve kept up all 3 certifications (IM, heme, medonc) through 2 full 10-year cycles of renewal, spending tens of thousands and countless hours./3

Wonderful study: 9/13 pts with ICI-related diabetes had germline mutations in NLRC5, with a significantly higher incidence than the general population. Toxicity biomarkers should be a priority of cancer research, particularly given the expanding use of IO in the curative setting.

Do I have this right? - ABIM requires yearly fees from physicians who are already cert to “maintain certification” - no evidence that MOC helps pt care - ABIM will report you as “not certified” if you don’t pay - hospitals require certification Idk kinda sounds like extortion
Nope. Not happening until I find out what this gets me , @ABIMFoundation. ⁩ What are we paying for? I’m certified…I took my boards and passed them.

1/ I watched Robert F. Kennedy Jr. on Joe Rogan yesterday and it gave me an opportunity to look into some of the "old-school" antivax claims. First Joe Rogan let's us know that he used to think RFK Jr. was "kinda loony", but now he's a true believer in RFK's antivax claims.
