CAR-T Cell Therapy | A New Dawn of Hope in Cancer Care
In a landmark leap in military medicine, Command Hospital, Chandimandir, #WesternCommand successfully administered its maiden CAR-T Cell Therapy – a revolutionary breakthrough that reprogrammes a patient’s own immune cells to seek and destroy cancer.
The life-saving therapy was administered to a 16-year-old patient battling relapsed refractory B-cell Acute Lymphoblastic Leukaemia, offering a new ray of hope.
Accomplished by the dedicated Hematology team of Western Healers, this milestone makes CH (WC) the first Command Hospital in the Armed Forces after Army Hospital (R&R), Delhi, to deliver this advanced therapy.
The achievement epitomises the fusion of technology, innovation and compassion, reaffirming the Indian Army's commitment to provide cutting-edge, world-class and humane care.

Can advanced cell therapies be delivered at scale without compromising quality?
That's one of the key questions explored in Dr. Atharva Karulkar's latest interview with @Citeline .
Drawing on the largest real-world CAR-T dataset from India, the discussion explores the science, manufacturing, logistics, and clinical outcomes behind NexCAR19, and why scalable access may become just as important as scientific innovation itself.
Some of the key insights from the conversation include:
• A 98% manufacturing success rate and a median vein-to-vein time of 29 days, enabled by a centralized manufacturing model and real-time logistics tracking.
• Real-world outcomes from 250 patients demonstrating efficacy comparable to globally approved CAR-T therapies in both leukemia and lymphoma.
• An approach to improving patient access through outcome-based financing and 0% EMI options, while working towards broader government and insurance inclusion.
• ImmunoACT's vision to expand manufacturing capacity, strengthen its global footprint, and make equitable access to CAR-T therapy a reality for patients beyond India.

What does a difficult-to-treat, real-world B-cell Non-Hodgkin Lymphoma (B-NHL) population actually look like?
In our previous publication spotlight, we explored the baseline characteristics of patients with relapsed/refractory B-ALL. This time, we turn to the 145 patients with relapsed/refractory B-NHL included in our study, published in Nature Portfolio's Blood Cancer Journal.
Like the B-ALL cohort, these patients reflect the realities of treating relapsed/refractory disease in routine clinical practice — physicians managing aggressive disease, extensive prior treatment histories, and limited therapeutic options.
Disease characteristics:
🔹 75% had primary refractory disease or an early relapse — an aggressive disease biology
🔹 80% had DLBCL & HGNHL histology — Diffuse Large B-Cell Lymphoma and High-Grade B-Cell Lymphoma, the most aggressive B-NHL subtypes
🔹 37% presented with bulky disease (tumour bulk ≥5 cm) prior to leukapheresis — associated with higher disease burden and treatment complexity
🔹 55% had extranodal disease — lymphoma involvement beyond the lymph nodes, reflecting more advanced disease
Treatment history:
🔹 83% had received 2+ prior lines of therapy — a heavily pretreated cohort
NexCAR19 was evaluated in this real-world, difficult-to-treat B-NHL population — representative of the patients clinicians encounter in everyday practice.

What does a difficult-to-treat, real-world B-cell Acute Lymphoblastic Leukemia (B-ALL) population actually look like?
Our study, published in @NaturePortfolio 's Blood Cancer Journal, reports baseline characteristics of 105 patients with relapsed/refractory B-ALL — drawn from a broader cohort of 250 patients — prior to treatment with NexCAR19 — the world's first humanized, and India's first, CAR-T cell therapy. We're sharing a look at that data here: a heavily pretreated, real-world population reflecting the challenges physicians face in routine practice.
Disease burden:
• More than half of patients had primary refractory disease or an early relapse — a subgroup historically associated with limited treatment options and poor prognosis
• 35% had high-blast leukemia (≥5% blasts) prior to leukapheresis, indicating clinically active disease at the time of treatment
• 2 out of 3 patients had Ph-like or Philadelphia chromosome-negative (Ph-ve) B-ALL — subtypes that lack the BCR-ABL1 alteration targeted by tyrosine kinase inhibitors in Ph-positive disease, making them harder to treat with standard therapy and more prone to relapse
• 10% had CNS involvement before leukapheresis, adding further clinical complexity
Treatment history:
• 3 in 4 patients had received two or more prior lines of therapy
• 34% had prior exposure to inotuzumab, reflecting a cohort already exposed to contemporary treatment approaches before CAR-T
Together, these characteristics define the real-world clinical context in which NexCAR19 was evaluated — a diverse, heavily pretreated population, not a curated trial cohort. Read the full paper to see how NexCAR19 delivered efficacy outcomes while also providing a favorable safety profile in ALL.
doi.org/10.1038/s41408-026-0…
How does this profile compare to the B-ALL patients you encounter — in the clinic or in your own research? We would love to know your perspective.

Developing NexCAR19, India's first indigenous CD19 CAR-T therapy was a scientific milestone. Delivering it to patients across the country proved just as demanding — and entirely different.
For a patient with relapsed blood cancer, access to treatment shouldn't depend on how far they live from a manufacturing site. Yet without the right infrastructure, that distance often determines who gets treated in time.
So, we set out to build a system to close that gap: a Centralized Coordinating Unit in Mumbai designed to deliver the same standard regardless of geography.
Our new publication in Nature Portfolio's Blood Cancer Journal presents the real-world data behind it: across 250 patients treated at 56 centres nationwide, ImmunoACT achieved a 98% manufacturing success rate — demonstrating that consistent, high-quality CAR-T care can be delivered reliably, irrespective of geography.
We're proud of what our team has built, and of what it means for patients everywhere who deserve access to the best that science can offer.

India's First Real-World Evidence for CAR-T Therapy, Now Published
We're proud to announce a landmark milestone — the publication of the first real-world evidence (RWE) on CAR-T therapy from India, now published in Blood Cancer Journal (Nature Portfolio | Impact Factor: 13.8), one of the world's leading hematology journals.
This study centres on NexCAR19, India's first indigenously developed CAR-T cell therapy. Drawing on real-world data from 250 patients treated across 56 centres, this is the first multicentre evaluation of CAR-T therapy in routine clinical practice in India. It marks a defining moment for cell therapy in the country: evidence that CAR-T can be delivered at scale, across diverse centres, with outcomes captured in real-world settings rather than the controlled environment of a trial.
To the authors and contributors — thank you.
This achievement belongs to the entire community behind it. Our deepest gratitude to the study authors, and to the clinicians, collaborating institutions, healthcare professionals, and researchers across all 56 centres whose expertise and dedication made this work possible. Above all, we thank the patients and caregivers who placed their trust in this therapy.
Over the coming weeks, we'll share key findings from the study and what they mean for the evolving landscape of CAR-T therapy in India.
