Cardiologist working for an academic university program; Helping to correct the myths of academic medicine and cardiology

One thing I have noticed in academic medicine - Any leader who is concerned about the size of their office is not somebody you want as your leader.

Did I miss something in medical school? Are patients with newly diagnosed DVT/PE told not to sleep? Is this correct?
#Eagles QB Jalen Hurts, who is out for the season after several blood clots were revealed in his lungs, has been undergoing tests for days. Before the London game, Hurts was in the hospital until 4 am and did not sleep because sleep was a risk. He played without having slept.

I have seen academic cardiologists who: --refuse to write Epic clinic notes (make NP do it) --refuse patient transfers when on service (so they do less work) --refuse to answer questions on outpatients when rounding b/c "too busy" --go out of town on admin days for errands
It's funny seeing some academic doctors complain that today's medical students and residents are too soft. Some of these docs see like 10 patients a week and don't come near a patient without an NP buffering zone.

Dumbest/funniest story? 70 year old w/ NICM was denied transfer by this MD from elsewhere for consideration for heart transplant as he is "over our age limit for transplant." Our program does not have an age cut-off for transplant and we transplanted a 72 year old last year.

Have you heard of elective ECMO cannulation? Transplant programs across USA are cannulating patients who meet hemodynamic criteria to give them the highest status for transplant. They are not crashing. Don't even have elevated lactate. "If you build it, they will game it"

Criteria are SBP < 90 mm Hg, elevated PCWP, and low cardiac index. That sounds like frank cardiogenic shock but many of these patients can be stabilized with IABP or Impella or even high dose inotropic support. But programs are going straight to ECMO to get the highest status

Grapes of Wrath should be read when on a trip out West. Great Gatsby should be read when you are in your 40s. Catch-22 should be read when you are fed up with the ignorance of hospital administrators.
Half the books you were "forced" to read in high school are actually spectacular, you just didn't have the emotional maturity to understand them yet.

Would it surprise you to know that Dr. Uriel received $50K from Abbott (the company making the only FDA approved LVAD on the market) in 2024/2025? openpaymentsdata.cms.gov/phy… I wonder how much that's going to be in 2026?
In young heart transplant candidates, is it best to pursue an LVAD-first approach or proceed directly to transplant? This JCF Focus Issue offers a debate, with perspective pieces for and against an LVAD-first approach.

At my previous institution the 2 highest h-index amongst cardiology faculty were: --an "interventional" cardiology thought leader who hadn't done a left heart cath in 10 years --A HF physician who showed up to work 2-3 days a week and couldn't write a note or formulate a plan.
The best physicians I know may not be “thought leaders”, on every national stage, or churning out nonsense publications every day, but they’re damn good clinicians and caring for patients.

Interesting how many times I ask a patient who referred them to see me for HF and the answer is a nurse practitioner Sad fact is many NPs are better at referring patients and identifying advanced HF than cardiologists This is how bad many general cardiologists are with HF.

Looking at the data, the worst performing programs all have leaders interested in writing papers, attending national meetings, making their mark on social media (Instagram, TikTok) NOT patient care.

Dumb things I saw when I rounded on HF service two weeks ago: --Neurology service recommending brain MRI for a LVAD patient with a stroke --CT surgery recommending viability study for a patient with multi-vessel CAD who is in ICU in cardiogenic shock

The move away from "hard" endpoints to surrogate ones would be applauded by big pharma and industry who can show their therapies "work" on the basis of some BS while patients show no benefit. The disclosures for this paper are an entire page! It's literally written by industry

Quadruple therapy is not utilized well in HF, so why are we talking about quintuple therapy? Javed Butler received $65,000 in 2025 from Merck, the company who makes vericiguat. How is he allowed to write an editorial supporting its use? Embarrassing openpaymentsdata.cms.gov/phy…
Replying to @JCardFail
Editorial perspective Quadruple GDMT remains underutilized. Even when achieved, substantial residual risk persists. Greene & Butler argue that vericiguat's safety, tolerability & ease of use make it an attractive option as part of "quintuple therapy". onlinejcf.com/article/S1071-…

Think of all the bullshit fellowships in cardiology that have been "developed" solely to generate papers: Cardio-oncology Cardiac amyloid Hypertrophic cardiomyopathy Sports cardiology
Replying to @PulmCrit
Eh. Programs are run by academics and want to train academics even though they know most won't be. I think that's the bigger reason why there is a focus on research

WVU performed 13 kidney and 8 heart transplants in 2025. Total 55 heart transplants in 8 years. Very low volumes for a hospital. Another example of the irrational transplant system in the US. There is no reason @WVUMedSchool should be involved in transplant at all @PghBizTimes
WVU Medicine welcomes Dr. Rakesh Sindhi as Executive Director of the Center for Organ Transplantation to expand lifesaving transplant care across the region. #WVUMedicine #OrganTransplant Read more:
