How does the move to PREVENT for ASCVD risk estimation affect aspirin use❓
📜jamanetwork.com/journals/jam…
1⃣ >80% eligible for 1ry prevention aspirin based on a PCE risk >10% would no longer be eligible w/ PREVENT
2⃣ Most current aspirin users don't qualify under either calculator
Do heart transplant patients always need invasive angio to screen for graft vasculopathy❓
📜authors.elsevier.com/a/1lWfA…
In this meta-analysis, CTA/PET had pretty respectable sensitivity/NPV for ruling out graft vasculopathy compared to a gold standard of invasive angiography.
Patients with a normal-ish EF could be further stratified as being at a *truly* low-risk of events or at intermediate risk depending on their rest flow.
In contrast, patients with a depressed EF had a uniformly high rate of events regardless.
Our hypothesis was that an elevated amount of myocardial blood flow at rest, which PET can measure, would predict adverse event rates.
And it did, esp. in patients with a normal EF at rest.
First, some background.
Prior data indicated that the myocardium of patients with LVH/HFpEF receives an excessive amount of blood flow at rest (doi.org/10.1161/CIRCHEARTFAI…)
There is also mechanistic data that medications effective in HFpEF reduce this (doi.org/10.2337/db20-0921)

In patients undergoing cardiac stress testing w/ PET, does rest flow matter❓
In an analysis of >8k patients, we find that it does, esp. in patients with a preserved EF.

How have HCM mortality rates changed before and after COVID ❓
1⃣ Prior to COVID, mortality rates declined by ~ half📉
2⃣ After COVID, mortality rates increased by ~30%, partially undoing some of the prior reductions in HCM mortality.📈

Further, imagers are now more likely to read multiple (3 or 4) modalities, with a small decline in single-modality imagers

How has cardiac imaging evolved in the US over the past decade❓

⏫Rapid rise in the rates of CT, PET, and CMR.
⏬In contrast, SPECT volumes have gone down:
